目的:探讨移植肾功能延迟恢复(DGF)的常见病因及其诊治方法。方法报告1例患者在肾移植术后出现持续时间长达2个月的驼峰状尿量变化,并对该病的相关文献进行复习,分析引起DGF的因素。结果该患者行同种异体肾移植术后尿量出现驼峰状变化,经给予静脉输入抗人T细胞猪免疫球蛋白,应用丹参川芎嗪注射液等药物改善循环,并规律血液透析治疗清除体内的炎症介质,维持水、电解质和酸碱平衡,预防感染和支持治疗后,患者尿量逐渐趋于稳定;波动于2000 mL左右,血肌酐稳定于130μmol/L左右后出院。结论充分认识DGF的常见病因有利于降低DGF的发生率,一旦发生应宜早期选择血液净化,辅助相应措施,绝大多数DGF可以恢复。
目的:探討移植腎功能延遲恢複(DGF)的常見病因及其診治方法。方法報告1例患者在腎移植術後齣現持續時間長達2箇月的駝峰狀尿量變化,併對該病的相關文獻進行複習,分析引起DGF的因素。結果該患者行同種異體腎移植術後尿量齣現駝峰狀變化,經給予靜脈輸入抗人T細胞豬免疫毬蛋白,應用丹參川芎嗪註射液等藥物改善循環,併規律血液透析治療清除體內的炎癥介質,維持水、電解質和痠堿平衡,預防感染和支持治療後,患者尿量逐漸趨于穩定;波動于2000 mL左右,血肌酐穩定于130μmol/L左右後齣院。結論充分認識DGF的常見病因有利于降低DGF的髮生率,一旦髮生應宜早期選擇血液淨化,輔助相應措施,絕大多數DGF可以恢複。
목적:탐토이식신공능연지회복(DGF)적상견병인급기진치방법。방법보고1례환자재신이식술후출현지속시간장체2개월적타봉상뇨량변화,병대해병적상관문헌진행복습,분석인기DGF적인소。결과해환자행동충이체신이식술후뇨량출현타봉상변화,경급여정맥수입항인T세포저면역구단백,응용단삼천궁진주사액등약물개선순배,병규률혈액투석치료청제체내적염증개질,유지수、전해질화산감평형,예방감염화지지치료후,환자뇨량축점추우은정;파동우2000 mL좌우,혈기항은정우130μmol/L좌우후출원。결론충분인식DGF적상견병인유리우강저DGF적발생솔,일단발생응의조기선택혈액정화,보조상응조시,절대다수DGF가이회복。
Objective To explore a common cause of delayed graft function(DGF)and its diagnosis and treatment methods. Methods One patient after renal transplantation were reported,who experience hump shape change of urine up to 2 months,and the related literature of the disease were reviewed toanalyze the causes of DGF. Results The allograft kidney transplantation patient show a hump shape change of urine,and was treated by porcine immunoglobulin of anti-human T cells,and Danshen and Chuanqongqin injection improving micro circulation. Regular hemodialysis was performed to remove the body of inflammatory mediators,and water, electrolyte and acid-base balance were maintained,and infection prevention and support treatment were done. Patient was discharged as urine output was gradually stabilized about 2 000 mL,and serum creatine around 130 μmol/L. Conclusion Full awareness of the common causes of DGF is helpful to reduce the incidence of DGF. In case of occurrence,early blood purification and auxiliary measures should be performed,and most DGF can recover.
