目的：探讨血小板抗体测定对反复输血超过3次者的临床价值。方法选择反复输血次数超过3次者72例，并选择同期输血次数在3次以内者69例，统计不同输血次数对血小板抗体阳性率的影响，计算在血小板输注后1h和输血后24h，血小板计数增加指数。结果输血次数在3次以上者，其发生血小板抗体阳性的比率显著高于输血次数在3次以内者（P＜0.05），血小板抗体阳性者输注血小板后1h和24h，其血小板增加指数均显著低于血小板抗体阴性者（P＜0.05）。结论对于既往输血次数超过3次者，进行血小板抗体检测，能更好的提高血小板输注效率，保证临床治疗效果，值得临床重视。
목적：탐토혈소판항체측정대반복수혈초과3차자적림상개치。방법선택반복수혈차수초과3차자72례，병선택동기수혈차수재3차이내자69례，통계불동수혈차수대혈소판항체양성솔적영향，계산재혈소판수주후1h화수혈후24h，혈소판계수증가지수。결과수혈차수재3차이상자，기발생혈소판항체양성적비솔현저고우수혈차수재3차이내자（P＜0.05），혈소판항체양성자수주혈소판후1h화24h，기혈소판증가지수균현저저우혈소판항체음성자（P＜0.05）。결론대우기왕수혈차수초과3차자，진행혈소판항체검측，능경호적제고혈소판수주효솔，보증림상치료효과，치득림상중시。
Objective To investigate the value of determination of platelet antibodies repeated blood for transfusions more than three times. Methods 72 cases with repeated transfusions over three times and 69 cases with repeated transfusions under three times were selected, the number of different impact on platelet transfusion antibody positive rate were statisticalled ,and platelet transfusion after 1 hour and 24 hours post-transfusion with increase in platelet count were calculated the index. Results The number of blood transfusions in the repeated transfusions over three times cases was more than with repeated transfusions under three times(P<0.05), the index increased platelet with platelet antibody positive after 1 hour and 24 hours were significantly lower than platelet antibody negative patients(P < 0.05). Conclusion Performed platelet antibody detection for the patients repeated transfusions over three times with the number of transfusions can better improve the efficiency of platelet transfusion, and ensure the clinical outcomes, so it is worthy of attention.
