目的:了解下呼吸道标本常见细菌的菌群分及耐药物征,为临床感染治疗提供参考依据。方法:采用湖南天地人半自动微生物鉴定仪和药敏系统进行菌株鉴定与药物敏感性测定,并检测ESBLS。结果:2011年1-12月我院临床分离1201株细菌中革兰阴性杆菌709(59.0%),革兰阳性球菌354株(29.5%)。革兰阴性杆菌中以铜绿假单胞菌占多数(162株),其余依次为大肠埃希菌(137株)肺炎克雷伯菌(85株)、鲍曼不动杆菌(12株)。金葡菌(SA)和凝固酶阴性葡萄球菌(CNS)中甲氧西林耐药株(MRSA和MRCNS)占45.1%,未发现耐万古霉素的葡球菌和肠球菌;大肠埃希菌和克雷伯菌属产ESBLs的发生率为50.4%和20.0%,革兰阴性杆菌对大多数抗菌药物(除了亚胺培南)耐药率高。结论:MRSA和MRCNS仍然是目前革兰氏阳性菌中主要问题,ESBLS菌株阳性率有增加趋势。对临床分离菌株的耐药情况应进行流行病学调查及采取相应控制措施,同时应合理应用抗生素并严密监控新的耐药菌的产生。

목적:료해하호흡도표본상견세균적균군분급내약물정,위림상감염치료제공삼고의거。방법:채용호남천지인반자동미생물감정의화약민계통진행균주감정여약물민감성측정,병검측ESBLS。결과:2011년1-12월아원림상분리1201주세균중혁란음성간균709(59.0%),혁란양성구균354주(29.5%)。혁란음성간균중이동록가단포균점다수(162주),기여의차위대장애희균(137주)폐염극뢰백균(85주)、포만불동간균(12주)。금포균(SA)화응고매음성포도구균(CNS)중갑양서림내약주(MRSA화MRCNS)점45.1%,미발현내만고매소적포구균화장구균;대장애희균화극뢰백균속산ESBLs적발생솔위50.4%화20.0%,혁란음성간균대대다수항균약물(제료아알배남)내약솔고。결론:MRSA화MRCNS잉연시목전혁란씨양성균중주요문제,ESBLS균주양성솔유증가추세。대림상분리균주적내약정황응진행류행병학조사급채취상응공제조시,동시응합리응용항생소병엄밀감공신적내약균적산생。
Obiective To investigate the distribution and antibiotic resistance of clinical bacteria isolated from inpatients in our hospital.Methods Bacterial identification and antibiotic susceptibility testing were earried out using MicroScanAutoScan-40 sys-tem.ESBLs production was also determined.Results A total of 1201 clinical isolates were analyzed,including 354strains of grampositive cocco and 709strains of gram-negativve bacilli.The prevalence of methicillin-resistant S.awreus and metjicillin-resistant coagulase-negative Staphylococcus was 45.1%respectively.No vancomycin-resistant Staphylococcus or Enterococcus strain was identified.ESBLs-producing strains accounted for 50.4% of E.coli and 20.0% of Klebsiella.The gram-negative bacilli strains were highly resistant resistant to antimicrobial agents(except imipenem).Conclusions:Methicillin-resistant staohylococcus (MRSA and MRCNS)is still the major proplem in gram-pointitive cocci. The rosiscant rates to ESBLS tended to increase.It’s wryently meeded to couduct epidemio-logical investigation and take measures to comtrol it mojocomial spread.It’s important to monitor the antimicrobial resistance of clinical pathogens.
