目的:观察高频通气(HFOV)联合珂立苏治疗足月儿呼吸窘迫综合征(RDS)的临床疗效。方法:66例重症足月的RDS患儿按随机数字表法分高频组和常频组,高频组43例,采用高频通气(HFOV)+珂立苏治疗;常频组23例,采用常频机械通气(CMV)治疗。对比两组治疗疗效及并发症情况。结果:两组上机后1、6、12 h的二氧化碳分压、吸氧浓度和氧合指数的比较差异有统计学意义(P<0.05);两组患儿的平均上机时间、用氧时间、住院天数、治愈率、死亡率及气胸发生率比较差异均有统计学意义(P<0.05)。结论:HFOV+珂立苏可以提高足月儿重症RDS治愈率,减少并发症。

목적:관찰고빈통기(HFOV)연합가립소치료족월인호흡군박종합정(RDS)적림상료효。방법:66례중증족월적RDS환인안수궤수자표법분고빈조화상빈조,고빈조43례,채용고빈통기(HFOV)+가립소치료;상빈조23례,채용상빈궤계통기(CMV)치료。대비량조치료료효급병발증정황。결과:량조상궤후1、6、12 h적이양화탄분압、흡양농도화양합지수적비교차이유통계학의의(P<0.05);량조환인적평균상궤시간、용양시간、주원천수、치유솔、사망솔급기흉발생솔비교차이균유통계학의의(P<0.05)。결론:HFOV+가립소가이제고족월인중증RDS치유솔,감소병발증。
Objective:To explore the effect of high frequency ventilation(HFOV)combined with Calsurf for full term respiratory distress syndrome(RDS). Method:66 with severe term respiratory distress syndrome were randomly divided into two groups according to random number table method. The control group was Chang Pin group,it had total 23 cases,they adopted Chang Pin mechanical ventilation(CMV). The treatment group was high-frequency group,it had 43 cases,they used high frequency ventilation(HFOV)+Calsurf. Compared the curative effect of two groups. Result:CO2 partial pressure,the comparison of oxygen concentration and oxygenation index had statistical significance in two groups(P<0.05);after using of breathing machine for one hour,six hours,twelve hours,the breathing machine application time on average ,oxygen time,hospitalization days,cure rate,mortality and incidence of pneumothorax had statistical significance of two groups(P<0.05). Conclusion:HFOV+Calsurf can improve the cure rate of full term severe RDS and reduce the complications.
