目的：探讨易漏诊及误诊的小肝癌（small hepatocellular carcinoma，SHCC）在螺旋CT扫描中的CT征象。方法分析漏诊或误诊的56例小肝癌CT征象，各病例均于首诊或复诊时经手术或穿刺活检病理证实。结果56例共发现61个病灶。其中漏诊19个病灶，误诊42个病灶。漏诊19个病灶中外生型12个病灶，其中具备典型CT征象7例，不典型CT征象5例，邻近均有肠管等其他影像干扰；另外7个病灶均呈不典型CT征象。误诊42个病灶均呈不典型CT征象。结论外生型小肝癌（肝脏脏面侧病灶）易漏诊，漏诊或误诊的小肝癌多为不典型CT征象（54/61）。
목적：탐토역루진급오진적소간암（small hepatocellular carcinoma，SHCC）재라선CT소묘중적CT정상。방법분석루진혹오진적56례소간암CT정상，각병례균우수진혹복진시경수술혹천자활검병리증실。결과56례공발현61개병조。기중루진19개병조，오진42개병조。루진19개병조중외생형12개병조，기중구비전형CT정상7례，불전형CT정상5례，린근균유장관등기타영상간우；령외7개병조균정불전형CT정상。오진42개병조균정불전형CT정상。결론외생형소간암（간장장면측병조）역루진，루진혹오진적소간암다위불전형CT정상（54/61）。
ObjectiveTo investigate the missed diagnosis and misdiagnosis of small hepatocellular carcinoma (SHCC) of CT signs in spiral CT scan.Method Analysis of 56 cases of small hepatocellular carcinoma CT signs of misdiagnosis or missed diagnosis, all cases were in the ifrst diagnosis or referral by operation or biopsy pathology.Results 56 cases were found in 61 lesions. The missed diagnosis in 19 lesions, 42 lesions misdiagnosed. Miss Jane 19 lesions and type 12 lesions, which have typical CT signs of 7 cases, 5 cases of atypical CT features, all other adjacent bowel image noise; the other 7 lesions were not typical CT signs. Misdiagnosis of 42 lesions were not typical CT signs.Conclusion Exogenous small hepatocellular carcinoma (liver dirty side lesions) missed: missed diagnosis or misdiagnosis of small hepatocellular carcinoma were not typical signs of CT (54/61).
