目的：探讨治疗胰腺损伤的术式选择及危险并发症的防治。方法：71例中29例行胰腺清创缝合引流术，17例行胰体尾切除、脾切除术，4例行胰体尾切除术，17例行胰体尾空肠Roux-Y式吻合术，2例行胰管吻合术。结果：71例中62例治愈，9例死亡（12.7％）。结论：Ⅲ～IV级胰腺损伤术后的严重并发症主要是胰肠吻合瘘，在术式上应选择切除远端胰腺及脾脏，避免胰肠Roux-Y式吻合术及胰管吻合。
목적：탐토치료이선손상적술식선택급위험병발증적방치。방법：71례중29례행이선청창봉합인류술，17례행이체미절제、비절제술，4례행이체미절제술，17례행이체미공장Roux-Y식문합술，2례행이관문합술。결과：71례중62례치유，9례사망（12.7％）。결론：Ⅲ～IV급이선손상술후적엄중병발증주요시이장문합루，재술식상응선택절제원단이선급비장，피면이장Roux-Y식문합술급이관문합。
Objective To explore proper surgical procedures and prevention of severe complications of pan-creatic injury. Methods In 71 cases of pancreatic injury, 29 cases received pancreatic debridement and drain-age, 17 cases received pancreatic body and tail resection with splenectomy, 4 cases received pancreatic body and tail resection. 17 cases were treated by Roux-Y type pancreaticojejunal anastomosis, and 2 cases re-ceived pancreatic duct anastomosis. Results Sixty two patients were cured, and 9 patients died (12.7%). Conclusion The major serious complications after operation of III~IV grade pancreatic injury are main- ly pancreaticojejunal anastomosis fistula. The distal part of the pancreas and spleen should be resected resolute-ly but not with the Roux-Y type pancreaticojejunal anastomosis or pancreatic duct anastomosis.
