Upper urinary tract stones%B ultrasound combined with CT positioning percutaneous nephrolithotomy reno ureteroscope lithotripsy
目的 探讨B超结合CT定位下经皮肾微造瘘输尿管镜下气压弹道碎石取石术治疗上尿路结石的临床疗效.方法 回顾性分析2011年10月至2013年5月收治的108例上尿路结石患者采用B超结合CT定位下经皮肾微造瘘输尿管镜下气压弹道碎石取石治疗,行一期或二期碎石.结果 一期穿刺碎石成功103例,其中有5例需二期碎石;5例转开放手术;无出现严重并发症及死亡病例.结论 采用B超结合CT定位经皮肾微造瘘输尿管镜下气压弹道碎石取石术治疗上尿路结石,临床建立皮肾通道简单可行,创伤性小,并发出血少;输尿管镜下气压弹道碎石取石疗效确切,碎石彻底,清除干净,结石残留少;无严重并发症等优越性,值得推广应用.

Objective To explore the clinical efficacy of B ultrasound combined with CT positioning percutaneous nephrolithotomy reno ureteroscopy pneumatic lithotripsy in the treatment of upper urinary calculi.Method Retrospectively analyzed 108 patients with upper urinary tract calculi in our hospital from October 2011 to May 2013,who underwent B ultrasound combined with CT positioning percutaneous nephrolithotomy reno ureteroscopy pneumatic lithotripsy,phase Ⅰ or Ⅱ gravel.Results There were 103 cases of successful phase Ⅰ puncture gravel,5 patients of whom required phase Ⅱ gravel; 5 cases turned to open surgery.There were no serious complications or death.Conclusions B ultrasound combined with CT guided percutaneous renal reno ureteroscopy pneumatic lithotripsy in the treatment of upper urinary tract stones is effective,with simple and feasible clinical percutaneous renal access,less trauma,less bleeding,thoroughly cleaned gravel,and less residual stones,without serious complications,worthy of clinical application.
