目的:为了优化妇科诊疗宫腔镜的清洗、消毒灭菌流程,探索经济、安全、快捷、高效灭菌方式。方法选取我院病人首次使用后污染的宫腔镜120件,随机分成3组, a 组40件(传统方法组):用传统方法清洗后浸泡于2%的普通戊二醛30分钟采样培养; B 组40件(优化流程组):酶+超声清洗干净后,用50ml 无菌注射器抽取75%乙醇20ml 腔内注射灌洗,腔外75%乙醇纱布擦拭消毒,保留5分钟后浸泡于2%的强化戊二醛30分钟采样培养; c 组40件(按规范清洗组):先用酶液浸泡后,再用超声清洗,浸泡于2%的强化戊二醛30分钟采样培养。结果 a 组清洗质量合格率为57.5%(23/40),灭菌合格率为52.5%(21/40);B 组清洗质量合格率为100%(40/40),灭菌合格率为100%(40/40);c 组清洗质量合格率为90%(36/40),灭菌合格率为80%(32/40);B 组的和 c 组的清洗质量高于 a 组,有统计学意义;B 组的灭菌合格率高于 a 组和 c 组.结论酶+超声清洗既可提高腔镜的清洗质量,还可缩短清洗的有效时间;75%乙醇腔内注射灌洗,腔外75%乙醇纱布擦拭消毒,;提高诊疗宫腔镜的灭菌合格率。此方法经济、高效、安全、可靠、特别适合临床应用。

목적:위료우화부과진료궁강경적청세、소독멸균류정,탐색경제、안전、쾌첩、고효멸균방식。방법선취아원병인수차사용후오염적궁강경120건,수궤분성3조, a 조40건(전통방법조):용전통방법청세후침포우2%적보통무이철30분종채양배양; B 조40건(우화류정조):매+초성청세간정후,용50ml 무균주사기추취75%을순20ml 강내주사관세,강외75%을순사포찰식소독,보류5분종후침포우2%적강화무이철30분종채양배양; c 조40건(안규범청세조):선용매액침포후,재용초성청세,침포우2%적강화무이철30분종채양배양。결과 a 조청세질량합격솔위57.5%(23/40),멸균합격솔위52.5%(21/40);B 조청세질량합격솔위100%(40/40),멸균합격솔위100%(40/40);c 조청세질량합격솔위90%(36/40),멸균합격솔위80%(32/40);B 조적화 c 조적청세질량고우 a 조,유통계학의의;B 조적멸균합격솔고우 a 조화 c 조.결론매+초성청세기가제고강경적청세질량,환가축단청세적유효시간;75%을순강내주사관세,강외75%을순사포찰식소독,;제고진료궁강경적멸균합격솔。차방법경제、고효、안전、가고、특별괄합림상응용。
Objective to optimize the cleaning,disinfection and sterilization process of gynecological diagnosis uteroscope, and explore an economic, security, and efficient sterilization method. Method 120 cases of uteroscopes after used were recruited in this study. these uteroscopes were randomly divided into three groups for sterilization with 40 cases in each group: Group A were first clean with traditional method and then soaked in 2% common glutaraldehyde for 30 minutes;Group B were using enzyme and ultrasonic for cleaning, followed by 75% alcohol lavaging intra cavitary, and then soaked in 2% common glutaraldehyde for 30 minutes; Group c were using enzyme and ultrasonic for cleaning, and then soaked in 2%common glutaraldehyde for 30 minutes; all the three groups were sampling and cultivation.Result the cleaning and sterilization rate in Group a were 57.5% (23/40) and 52.5% (21/40) respectively; while both of these two rate in group B were 100%(40/40); However, the cleaning and sterilization rate in Group a were 90% (36/40) and 80% (32/40). after statistical analyzed, the cleaning rate in Group B and C were significant higher than that in Group A, Howerver, the sterilization rate in Group B was significant higher than that in group A and group C.Conclusion enzyme and ultrasonic cleaning can not only improve the uteroscope cleaning quality , but also shorten cleaning time ; 75% alcohol lavaging intracavitary can rapidly improve the sterilization rate of diagnostic uteroscope sterilization. This method is economical, efficient, safe, reliable, and especially suitable for clinical application.
