目的：探索70岁以上肺癌患者施行手术切除的手术适应症及并发症的预防与治疗策略。方法分析我院自2003年1月至2012年12月行肺癌切除术266例。其中70岁以上患者63例，男性41例，女性22例。年龄70～75岁者41例，75～80岁者22例。行肺叶切除术25例，楔形切除术30例，肺段切除术8例。结果术后并发症死亡3例，1例死于心肌梗死，1例死于呼吸衰竭，1例死于肺栓塞。结论年龄大于70岁以上者行肺癌手术，术后并发症死亡率4.76%（3人/63人），明显高于同期的70岁以下病例的1.5%。但是，严格把握手术病例适应症，充分地做好术前准备，尤其注意肺功能和心脏功能检查和术前对心肺功能的改善，加强术后心肺的管理，积极预防并处理心肺并发症。可以明显减少术后并发症和手术死亡率。从而提高70岁以上肺癌患者的手术治疗效果。
목적：탐색70세이상폐암환자시행수술절제적수술괄응증급병발증적예방여치료책략。방법분석아원자2003년1월지2012년12월행폐암절제술266례。기중70세이상환자63례，남성41례，녀성22례。년령70～75세자41례，75～80세자22례。행폐협절제술25례，설형절제술30례，폐단절제술8례。결과술후병발증사망3례，1례사우심기경사，1례사우호흡쇠갈，1례사우폐전새。결론년령대우70세이상자행폐암수술，술후병발증사망솔4.76%（3인/63인），명현고우동기적70세이하병례적1.5%。단시，엄격파악수술병례괄응증，충분지주호술전준비，우기주의폐공능화심장공능검사화술전대심폐공능적개선，가강술후심폐적관리，적겁예방병처리심폐병발증。가이명현감소술후병발증화수술사망솔。종이제고70세이상폐암환자적수술치료효과。
Objective research surgical indications and prevention and treatment strategies of complications among patients over 70 years old who underwent operation of lung cancer. Methods analysis the data of 266 patients who underwent pneumonectomy from January 2003 to december 2012 in our hospital. there are 63 patients over 70 years old ,41 are males while 22 are females. 41 cases between 70 to 75 years old, 22 cases between 75 to 80 years old. 25 cases underwent pulmonary lobectomy , 30 cases underwent wedge resection and 8 cases underwent segmentectomy. Result 3 patients died of postoperative complications, one died of myocardial infarction, 1 died of respiratory failure, and 1 died of pulmonary embolism. Conclusion the mortality rate of postoperative complications in patients over 70 years old who underwent operation for lung cancer is 4.76%(3/63), significantly higher than the 1.5% in the same period of younger than 70 years old cases. However, strictly assurance the surgical indications, adequately prepare before operation, especially the function examination for heart and lung, strengthen postoperative management, and positively prevent and treat complications of heart and lung could significantly decrease the incidence of complication and mortality rate. following above could elevate the treatment effectiveness of patients with lung cancer over 70 years old.
