目的:观察新生儿缺氧缺血性脑病(HIE)患儿治疗前后血清白介素-18(IL-18)、白介素-6(IL-6)和干扰素-γ( IFN-γ)浓度的变化,了解IL-18、IL-6、IFN-γ与HIE的关系。方法收集98例HIE患儿(轻度40例,中度32例,重度26例)治疗前、治疗后2周及42名健康新生儿(对照组)血清,采用酶联免疫吸附法( ELISA)检测血清IL-18、IL-6和IFN-γ浓度,并对HIE患儿治疗前IL-18与IL-6、IFN-γ的相关性进行分析。结果轻度、中度、重度HIE患儿治疗前血清IL-18、IL-6和IFN-γ浓度较对照组明显增高(P<0.05),且HIE病情分度越重,血清IL-18、IL-6和IFN-γ浓度增加越明显(P<0.05);HIE患儿治疗前IL-18与IL-6、IFN-γ均呈显著正相关(P<0.05);HIE患儿治疗后血清IL-18、IL-6和IFN-γ浓度较治疗前降低(P<0.05)。结论IL-18、IL-6、IFN-γ可能参与HIE发病过程,血清IL-18、IL-8、IFN-γ水平可作为HIE病情判断及疗效的观察指标。

목적:관찰신생인결양결혈성뇌병(HIE)환인치료전후혈청백개소-18(IL-18)、백개소-6(IL-6)화간우소-γ( IFN-γ)농도적변화,료해IL-18、IL-6、IFN-γ여HIE적관계。방법수집98례HIE환인(경도40례,중도32례,중도26례)치료전、치료후2주급42명건강신생인(대조조)혈청,채용매련면역흡부법( ELISA)검측혈청IL-18、IL-6화IFN-γ농도,병대HIE환인치료전IL-18여IL-6、IFN-γ적상관성진행분석。결과경도、중도、중도HIE환인치료전혈청IL-18、IL-6화IFN-γ농도교대조조명현증고(P<0.05),차HIE병정분도월중,혈청IL-18、IL-6화IFN-γ농도증가월명현(P<0.05);HIE환인치료전IL-18여IL-6、IFN-γ균정현저정상관(P<0.05);HIE환인치료후혈청IL-18、IL-6화IFN-γ농도교치료전강저(P<0.05)。결론IL-18、IL-6、IFN-γ가능삼여HIE발병과정,혈청IL-18、IL-8、IFN-γ수평가작위HIE병정판단급료효적관찰지표。
Objective To observe the changes of serum interleukin-18 ( IL-18 ) , interleukin-6 ( IL-6 ) and interferon-γ ( IFN-γ) concentrations in newborns with hypoxic-ischemic encephalopathy ( HIE) so as to understand the significance of changes of IL-18, IL-6, and IFN-γ in HIE. Methods The serum of 98 child patients with HIE ( including 40 mild, 32 moderate and 26 severe cases) as well as 42 healthy newbo-rns (control group) was collected 2 weeks before and after treatment. The serum concentrations of IL-18, IL-6 and IFN-γ were detected by enzyme linked immunosorbent assay ( ELISA) . Results In comparison with control group, serum IL-18, IL-6 and IFN-γconcentrations in mild, moderate and severe HIE cases before treatment were significantly increased (P<0. 05). Moreover, with the increasing severity of HIE, ser-um concentrations of IL-18 , IL-6 and IFN-γwere growing increased ( P<0 . 05 ) . There were positive cor-relation between IL-18 and IL-6 , and IFN-γ in HIE child patients before treatment ( r=0 . 72 , 0 . 75;P<0 . 05 ) . Serum concentrations of IL-18 , IL-6 and IFN-γ were significantly reduced in HIE group before treatment comparing with after treatment ( P<0 . 05 ) . Conclusion IL-18 , IL-6 and IFN-γ may participate in the pathogenesis of HIE. Serum IL-18, IL-8 and IFN-γconcentrations could be regard as novel indica-tors to evaluate the severity and prognosis of HIE.
