目的：探讨全胸腔镜肺叶切除治疗非小细胞肺癌的疗效。方法：2010年11月至2012年11月，我院共施行198例全胸腔镜下肺叶切除术，肺叶三大结构切除顺序无固定模式，以手术方便安全为准。结果：198例患者手术顺利，无严重并发症及围手术期死亡病例。中转开胸6例。手术时间220.2±15.6min，术中出血160.4±43.0ml，每例清扫纵隔淋巴结3~7组。术后拔管时间4±2.2天。术后病理：腺癌124例，鳞癌65例，腺鳞癌8例，肉瘤样癌1例。1例Ⅲ期患者术后17个月死亡，其他197例患者随访2~24个月无复发转移。结论：全胸腔镜手术肺叶切除创伤小、术后疼痛轻、术后拔管时间短、住院时间短，在肺癌治疗领域会有更大的发展。
목적：탐토전흉강경폐협절제치료비소세포폐암적료효。방법：2010년11월지2012년11월，아원공시행198례전흉강경하폐협절제술，폐협삼대결구절제순서무고정모식，이수술방편안전위준。결과：198례환자수술순리，무엄중병발증급위수술기사망병례。중전개흉6례。수술시간220.2±15.6min，술중출혈160.4±43.0ml，매례청소종격림파결3~7조。술후발관시간4±2.2천。술후병리：선암124례，린암65례，선린암8례，육류양암1례。1례Ⅲ기환자술후17개월사망，기타197례환자수방2~24개월무복발전이。결론：전흉강경수술폐협절제창상소、술후동통경、술후발관시간단、주원시간단，재폐암치료영역회유경대적발전。
Objective:To explore the completely thoracoscopic lobectomy for non-smal cel lung cancer.Methods:from November 2010 to November 2012, 198 cases were performed in our hospital completely thoracoscopic lobectomy, lung resection order three structures no fixed pat ern, whichever is convenient and safe operation. Results:of 198 cases surgery was uneventful, with no serious complications and perioperative deaths. Thoracotomy in 6 patients. The operative time 220.2 ± 15.6min, bleeding 160.4 ± 43.0ml, mediastinal lymph node dissection in each case 3 to 7 groups. Postoperative extubation time 4 ± 2.2 days. 124 cases of adenocarcinoma, 65 cases of squamous cel carcinoma ,8 cases of adenosquamous carcinoma.One caseⅢpatients died after 17 months,Other 197 cases were fol owed up for 2 to 24 months without recurrence. Conclusion:completely thoracoscopic surgery lobectomy trauma, postoperative pain, postoperative extubation time is short, shorter hospital stay, in the field of lung cancer have a greater development.
