目的:研究全自动毛细管电泳仪检测血红蛋白A2(HbA2)在β地中海贫血筛查中的临床应用价值。方法986例需进行地贫筛查者,分别采用全自动毛细管电泳和琼脂糖凝胶电泳进行血红蛋白A2检测,并与基因分析结果进行对比,分析两种方法的灵敏度和特异度。结果以HbA2≥4.8%,毛细管电泳诊断β地贫灵敏度和特异度分别为82.3%和87.6%,琼脂糖凝胶电泳诊断β地贫灵敏度和特异度分别为77.6%和81.7%,毛细管电泳对β地中海贫血诊断的灵敏度和特异度较高。结论毛细管电泳比琼脂糖电泳能更准确区分HbA2、HbF等条带,对β地贫筛查效果更佳。

Objective To study clinical application value of detecting hemoglobin A2 by automatic capillary electrophoresis in β-thalassemia screening. Methods A total of 986 cases of patients required β-thalassemia screening were selected and de-tected the hemoglobin A2 by fully automatic capillary electrophoresis and agarose gel electrophoresis separately. Analysising the sensitivity and specificity of the two methods, and the results were compared with the results detected by gene analysis. Results If HbA2≥4.8%, the sensitivity and specificity of capillary electrophoresis were 82.3% and 87.6% respectively in β-thalassemia detection, the sensitivity and specificity of agarose gel electrophoresis were 77.6%and 81.7%respectively in β-thalassemia detec-tion.The sensitivity and specificity of capillary electrophoresis were higher than that of agarose gel electrophoresis in βthalassemia diagnosis. Conclusion The capillary electrophoresis is better than agarose gel electrophoresis to distinguish the HbA2 ,HbF and other bands inβ-thalassemia screening.
