目的:探讨血浆N端脑利钠肽前体(N-terminal precursor of brain-type natriuretic peptide,NT-proBNP)在老年慢性充血性心力衰竭(chronic congestive heart failure,CHF)诊断中的价值及与心功能的关系。方法选择83例CHF患者和60例健康体检者,采用化学发光免疫分析法检测血浆NT-proBNP水平,超声心动图检测患者心脏结构和功能改变。结果 CHF组血浆NT-proBNP水平(1643.3±756.7 pg/ml)显著高于健康体检者(46.2±11.4 pg/ml),差异有统计学意义(P<0.05);而且,随着病情的进展,血浆NT-proBNP含量逐渐升高(P<0.05)。结论血浆NT-proBNP水平与CHF患者的心力衰竭程度正相关,在临床CHF病人诊断和分级中具有重要的作用。

목적:탐토혈장N단뇌리납태전체(N-terminal precursor of brain-type natriuretic peptide,NT-proBNP)재노년만성충혈성심력쇠갈(chronic congestive heart failure,CHF)진단중적개치급여심공능적관계。방법선택83례CHF환자화60례건강체검자,채용화학발광면역분석법검측혈장NT-proBNP수평,초성심동도검측환자심장결구화공능개변。결과 CHF조혈장NT-proBNP수평(1643.3±756.7 pg/ml)현저고우건강체검자(46.2±11.4 pg/ml),차이유통계학의의(P<0.05);이차,수착병정적진전,혈장NT-proBNP함량축점승고(P<0.05)。결론혈장NT-proBNP수평여CHF환자적심력쇠갈정도정상관,재림상CHF병인진단화분급중구유중요적작용。
Objective To explore the diagnosis value of plasma N-terminal precursor of brain-type natriuretic peptide (NT-proBNP) in elderly patients with chronic congestive heart failure (CHF) patients and its relation with cardiac function. Methods The levels of plasma NT-proBN were detected by Chemiluminescence Immunoassay in 83 patients with CHF and 60 healthy con-trols. The changes of heart structure and function were measured by echocardiogram. Results The level of NT-proBNP in CHF group (1643.3±756.7 pg/ml) was significantly higher than that in health control group (46.2±11.4 pg/ml) (P<0.05). With the aggra-vation of CHF, the levels of NT-proBNP increased gradually (P<0.05). Conclusion The level of plasma NT-proBNP is positively correlated with the degree of CHF, and may play an important role in clinical diagnosis and classification of CHF.
