_目的:探讨磁共振波谱(MRS)对胰腺癌的诊断价值。方法:应用单体素点分辨波谱(PRESS)序列,采集53例非胰腺癌受检者(包括26例正常胰腺,16例急性胰腺炎,11例慢性胰腺炎)及22例胰腺癌患者的胰腺质子磁共振波谱(1 H-MRS)图像。通过分析非癌组与癌组胰腺波谱中代谢物的表现,比较两组的代谢物峰显示率,计算每组胰腺波谱中胆固醇和不饱和脂肪酸的混合峰(Chol+Unsat)与脂肪峰(Lip)峰高的比值(Chol+Unsat/Lip),在胰腺癌与非胰腺癌的比值之间进行受试者工作特征(ROC)曲线分析。结果:胰腺1 H-MRS 显示的主要代谢物峰有 Lip 峰、Chol+Unsat 峰和胆碱(Cho)峰。胰腺癌组与非胰腺癌组的 Chol+Unsat/Lip 比值分别为0.181±0.059和0.328±0.111,两组比值差异有统计学意义(r=2.913,P=0.008)。经计算得出 ROC 曲线下面积为0.857(P=0.000)。当 Chol+Unsat/Lip 比值临界值取0.286时,1 H-MRS 鉴别诊断胰腺癌与非胰腺癌的敏感度为68.2%,特异度为100%。结论:胰腺1 H-MRS 中 Chol+Un-sat/Lip 比值升高对诊断胰腺癌有一定价值。

_목적:탐토자공진파보(MRS)대이선암적진단개치。방법:응용단체소점분변파보(PRESS)서렬,채집53례비이선암수검자(포괄26례정상이선,16례급성이선염,11례만성이선염)급22례이선암환자적이선질자자공진파보(1 H-MRS)도상。통과분석비암조여암조이선파보중대사물적표현,비교량조적대사물봉현시솔,계산매조이선파보중담고순화불포화지방산적혼합봉(Chol+Unsat)여지방봉(Lip)봉고적비치(Chol+Unsat/Lip),재이선암여비이선암적비치지간진행수시자공작특정(ROC)곡선분석。결과:이선1 H-MRS 현시적주요대사물봉유 Lip 봉、Chol+Unsat 봉화담감(Cho)봉。이선암조여비이선암조적 Chol+Unsat/Lip 비치분별위0.181±0.059화0.328±0.111,량조비치차이유통계학의의(r=2.913,P=0.008)。경계산득출 ROC 곡선하면적위0.857(P=0.000)。당 Chol+Unsat/Lip 비치림계치취0.286시,1 H-MRS 감별진단이선암여비이선암적민감도위68.2%,특이도위100%。결론:이선1 H-MRS 중 Chol+Un-sat/Lip 비치승고대진단이선암유일정개치。
To study the value of 3.0T 1 H-MRS in the diagnosis of pancreatic cancer.Methods:Spectrosco-pies of pancreas in 75 subjects were acquired with single voxel PRESS sequence on a 3.0T MR scanner.The subjects includ-ed normal pancreas (n=26),acute pancreatitis (n=16),chronic pancreatitis (n=11)and pancreatic cancer (n=22).The spectral appearances were analyzed,and metabolites visibility rates between pancreatic cancer and non-cancer group were compared.The ratio of cholesterol and the olefinic region of fatty acids (Chol +Unsat)peak height to lipid (Lip)peak height (Chol+Unsat/Lip)was calculated in each 1 H-MRS,and a ROC analysis was performed between the ratios of two groups.Results:There were three major resonance peaks in the spectra:Lip,Chol+Unsat,and choline.The mean value of Chol+Unsat/Lip in 1 H-MRS was 0.181±0.059 for non-cancer group,and 0.328±0.111 for pancreatic cancer group.Sta-tistically significant difference was found in the values of Chol+Unsat/Lip ratios between cancer and non-cancer group (r=2.913,P=0.008).The area under the ROC curve was 0.857 (P=0.000).With ROC cut point setting to 0.286,the diag-nostic sensitivity and specificity for pancreatic cancer were 68.2% and 85.9%,respectively.Conclusion:1 H-MRS of pancre-atic cancer has a higher ratio of Chol+Unsat/Lip,and this may be useful in differentiating pancreatic cancer from other pan-creatic diseases.
