_目的:总结分析幕上脑实质内室管膜瘤的 MRI 及 CT 表现,并与病理分级进行对照分析,以提高诊断符合率。方法:对经手术病理证实的18例幕上脑实质内室管膜瘤的影像及病理表现进行回顾性分析,18例均行颅脑 CT 或MRI 检查。结果:7例病理分级为Ⅱ级,病灶多较小,位置多表浅;11例病理分极为Ⅲ级,病灶多贴近侧脑室;17例病变为囊、实性占位,其中16例囊变位于病灶周边,当病灶较大时,较大囊变部分多位于脑深部;18例病变均呈混杂性长 T1、长T2信号改变,实质部分可见多发点状低信号灶,实性部分 DWI 呈稍高信号;其中5例行 CT 检查者病变呈稍高密度。14例扫描呈不均匀、分房分隔样强化;病灶周围的水肿程度与病理分级没有明确相关性。结论:幕上脑实质内室管膜瘤的MRI 表现有一定的特征性,CT 检查对提高诊断符合率有一定价值。
_目的:總結分析幕上腦實質內室管膜瘤的 MRI 及 CT 錶現,併與病理分級進行對照分析,以提高診斷符閤率。方法:對經手術病理證實的18例幕上腦實質內室管膜瘤的影像及病理錶現進行迴顧性分析,18例均行顱腦 CT 或MRI 檢查。結果:7例病理分級為Ⅱ級,病竈多較小,位置多錶淺;11例病理分極為Ⅲ級,病竈多貼近側腦室;17例病變為囊、實性佔位,其中16例囊變位于病竈週邊,噹病竈較大時,較大囊變部分多位于腦深部;18例病變均呈混雜性長 T1、長T2信號改變,實質部分可見多髮點狀低信號竈,實性部分 DWI 呈稍高信號;其中5例行 CT 檢查者病變呈稍高密度。14例掃描呈不均勻、分房分隔樣彊化;病竈週圍的水腫程度與病理分級沒有明確相關性。結論:幕上腦實質內室管膜瘤的MRI 錶現有一定的特徵性,CT 檢查對提高診斷符閤率有一定價值。
_목적:총결분석막상뇌실질내실관막류적 MRI 급 CT 표현,병여병리분급진행대조분석,이제고진단부합솔。방법:대경수술병리증실적18례막상뇌실질내실관막류적영상급병리표현진행회고성분석,18례균행로뇌 CT 혹MRI 검사。결과:7례병리분급위Ⅱ급,병조다교소,위치다표천;11례병리분겁위Ⅲ급,병조다첩근측뇌실;17례병변위낭、실성점위,기중16례낭변위우병조주변,당병조교대시,교대낭변부분다위우뇌심부;18례병변균정혼잡성장 T1、장T2신호개변,실질부분가견다발점상저신호조,실성부분 DWI 정초고신호;기중5례행 CT 검사자병변정초고밀도。14례소묘정불균균、분방분격양강화;병조주위적수종정도여병리분급몰유명학상관성。결론:막상뇌실질내실관막류적MRI 표현유일정적특정성,CT 검사대제고진단부합솔유일정개치。
To analyze MRI and CT features of supratentorial intra-cerebral ependymoma in order to im-prove diagnosis of this disease.Methods:The CT and MRI data of 18 cases with supratentorial intra-cerebral ependymoma, which were confirmed by postoperative pathology,were analyzed retrospectively.Results:7 cases were classified as grade Ⅱpathologically,showing that the lesions were relative smaller and located at the superfical part of the brain,while the other 11 cases were classified as grade Ⅲ,demonstrating that lesions were relatively bigger,and deeper in white matter close to the lateral ventricle;Lesions in 17 cases were cystic-solid type,16 of which showed cystic degeneration in periphery regions of the lesions.If the lesion was big,most of larger cysts located in the deep area of cerebral white matter;All the lesions showed hypo-intensity on T1 WI and hyper-intensity on T2 WI,and hypo-intense spots can be founded within the parenchy-mal part.The solid portion demonstrated high signal on DWI.Lesions in 5 cases showed hyperattenuation on CT scan.Post-contrast MR image of 14 cases revealed inhomogeneous enhancement of the solid portion and obious septum enhancement;There was no definite correlation between extent of perifocal edema and the pathological grade.Conclusion:Intra-cerebral ependymomas have several relatively characteristic MRI features.CT examination has definite value for improving diagnostic accuracy.
