目的:应用动态轮廓眼压计( dynamic contour tonometer , DCT)测量正常眼压性青光眼( normal tension glaucoma , NTG)及原发性开角型青光眼( primary open angle glaucoma , POAG)患者的眼压( intraocular pressure , IOP)及眼脉动幅度值( ocular pulse amplitude , OPA),并探讨影响OPA测量值的相关因素。<br> 方法:选取NTG患者20例,POAG患者21例及正常对照组各20例,应用Goldmann压平眼压( Goldmann applanation tonometer, GAT)及DCT测量IOP,同时测量了中央角膜厚度(central corneal thickness,CCT)、眼轴(axial length, AL)、心率( heart rate , HR )、收缩压( systolic blood pressure , SBP)、舒张压( diastolic blood pressure ,DBP)等指标,并探讨GAT,DCT两种眼压计测量的相关性,进而分析OPA测量值的影响因素。<br> 结果:三组被检者的IOP,OPA,SBP 及DBP差异均有统计学意义(P<0.05),而CCT,AL及HR差异无统计学意义(P>0.05)。其中OPA值NTG组为(1.7±0.9)mmHg,POAG组为(2.8±0.7)mmHg,正常对照组为(2.4±0.6)mmHg;NTG组与POAG组比较( P=0.001),与正常对照组比较( P=0.005)差异均有统计学意义,POAG组与正常对照组比较(P=0.502)差异无统计学意义;OPA值与IOP,HR及CCT无相关性(P>0.05),与AL,SBP及DBP具有一定的相关性(P<0.05)。<br> 结论:NTG患者与POAG患者及正常人的OPA值存在差异,同时OPA值受AL及SBP及DBP影响。

목적:응용동태륜곽안압계( dynamic contour tonometer , DCT)측량정상안압성청광안( normal tension glaucoma , NTG)급원발성개각형청광안( primary open angle glaucoma , POAG)환자적안압( intraocular pressure , IOP)급안맥동폭도치( ocular pulse amplitude , OPA),병탐토영향OPA측량치적상관인소。<br> 방법:선취NTG환자20례,POAG환자21례급정상대조조각20례,응용Goldmann압평안압( Goldmann applanation tonometer, GAT)급DCT측량IOP,동시측량료중앙각막후도(central corneal thickness,CCT)、안축(axial length, AL)、심솔( heart rate , HR )、수축압( systolic blood pressure , SBP)、서장압( diastolic blood pressure ,DBP)등지표,병탐토GAT,DCT량충안압계측량적상관성,진이분석OPA측량치적영향인소。<br> 결과:삼조피검자적IOP,OPA,SBP 급DBP차이균유통계학의의(P<0.05),이CCT,AL급HR차이무통계학의의(P>0.05)。기중OPA치NTG조위(1.7±0.9)mmHg,POAG조위(2.8±0.7)mmHg,정상대조조위(2.4±0.6)mmHg;NTG조여POAG조비교( P=0.001),여정상대조조비교( P=0.005)차이균유통계학의의,POAG조여정상대조조비교(P=0.502)차이무통계학의의;OPA치여IOP,HR급CCT무상관성(P>0.05),여AL,SBP급DBP구유일정적상관성(P<0.05)。<br> 결론:NTG환자여POAG환자급정상인적OPA치존재차이,동시OPA치수AL급SBP급DBP영향。
AIM: To survey the application of dynamic contour tonometer ( DCT ) in normal tension glaucoma ( NTG ) patients, primary open angle glaucoma(POAG) patients and healthy controls. And to analyze the factors that influence the ocular pulse amplitude ( OPA) . <br> METHODS: In this case-control study, twenty NTG patients, twenty-one POAG patients and twenty health people were included in the study.Intraocular pressure ( IOP) was measured in each group with GAT and DCT. Correlation between the value of IOP measured with DCT and GAT were analyzed. Meanwhile, central corneal thickness ( CCT ) , axial length ( AL ) , systolic blood pressure(SBP), diastolic blood pressure(DBP) and heart rate( HR) were also measured to evaluate the influencing factor of OPA. <br> RESULTS: There were significant statistical differences of IOP, OPA, SBP and DBP among NTG patients, POAG patients and healthy controls.There were no significant statistical differences of CCT, AL and HR among those subjects.The OPA were 1.7 ±0.9mmHg in NTG patients, 2.8 ±0.7mmHg in POAG patients and 2.4 ±0.6mmHg in normal controls. There was no significant statistical difference of OPA between POAG patients and normal controls(P=0.502), whereas significant statistical difference of OPA between POAG patients and NTG patients (P=0.001) was observed. Significant statistical difference of OPA between NTG patients and normal controls(P=0.005) was detected. The results of Pearson correlation analysis showed that there was significant correlation between OPA with AL, SBP and DBP ( P<0.05 ).No significant correlation was found between OPA with IOP, HR and CCT(P>0.05). <br> CONCLUSION: There are differences of OPA among NTG patients, PAOG patients and normal controls.And OPA is influenced by AL, SBP and DBP.
