目的:研究脑梗死患者的心脏自主神经功能及心功能变化情况,以评估病情及预后。方法检测我院首发急性脑梗死的89例患者(观察组)及80例健康体检者(对照组)的心率变异性(HRV)及左室射血分数(LVEF),并观察急性脑梗死发生不同部位心电图发生改变的情况及预后,同时比较分析两组受检者的心率变异性及左室射血分数检测值。结果急性脑梗死患者中,心电图变化的发生率为65.1%,脑干病变的心电图改变发生率显著高于基底节、丘脑病变及脑叶、皮质病变(P <0.05),而基底节、丘脑病变的心电图改变发生率也略高于脑叶、皮质病变。且观察组的HF、LF、TF、SDNN、RMSSD、LVEF均低于对照组(P <0.05)。同时,4例心电图发生变化的脑干病变患者在观察期间死亡,其余心电图发生变化的患者预后较未出现心电图变化者较差,HRV各项指标及LVEF值降低明显。结论梗死病灶距离心血管中枢越近,心电图发生改变的几率越大,且HRV各项指标及EF值随着脑梗死病情的加重而逐渐下降。
目的:研究腦梗死患者的心髒自主神經功能及心功能變化情況,以評估病情及預後。方法檢測我院首髮急性腦梗死的89例患者(觀察組)及80例健康體檢者(對照組)的心率變異性(HRV)及左室射血分數(LVEF),併觀察急性腦梗死髮生不同部位心電圖髮生改變的情況及預後,同時比較分析兩組受檢者的心率變異性及左室射血分數檢測值。結果急性腦梗死患者中,心電圖變化的髮生率為65.1%,腦榦病變的心電圖改變髮生率顯著高于基底節、丘腦病變及腦葉、皮質病變(P <0.05),而基底節、丘腦病變的心電圖改變髮生率也略高于腦葉、皮質病變。且觀察組的HF、LF、TF、SDNN、RMSSD、LVEF均低于對照組(P <0.05)。同時,4例心電圖髮生變化的腦榦病變患者在觀察期間死亡,其餘心電圖髮生變化的患者預後較未齣現心電圖變化者較差,HRV各項指標及LVEF值降低明顯。結論梗死病竈距離心血管中樞越近,心電圖髮生改變的幾率越大,且HRV各項指標及EF值隨著腦梗死病情的加重而逐漸下降。
목적:연구뇌경사환자적심장자주신경공능급심공능변화정황,이평고병정급예후。방법검측아원수발급성뇌경사적89례환자(관찰조)급80례건강체검자(대조조)적심솔변이성(HRV)급좌실사혈분수(LVEF),병관찰급성뇌경사발생불동부위심전도발생개변적정황급예후,동시비교분석량조수검자적심솔변이성급좌실사혈분수검측치。결과급성뇌경사환자중,심전도변화적발생솔위65.1%,뇌간병변적심전도개변발생솔현저고우기저절、구뇌병변급뇌협、피질병변(P <0.05),이기저절、구뇌병변적심전도개변발생솔야략고우뇌협、피질병변。차관찰조적HF、LF、TF、SDNN、RMSSD、LVEF균저우대조조(P <0.05)。동시,4례심전도발생변화적뇌간병변환자재관찰기간사망,기여심전도발생변화적환자예후교미출현심전도변화자교차,HRV각항지표급LVEF치강저명현。결론경사병조거리심혈관중추월근,심전도발생개변적궤솔월대,차HRV각항지표급EF치수착뇌경사병정적가중이축점하강。
Objective To evaluate the prognosis and state of illness by studying the cardiac autonomic nervous function and heart function in patients with acute cerebral infarction. Methods HRV and LVEF of 89 patients with the first onset of acute cerebral infarction(observation group) and 80 healthy people(control group) were detected, ECG changes of different parts and prognosis in all patients were observed, HRV and LVEF of two groups were analyzed comparatively. Results ECG change rate was 65.1% in patients with acute cerebral infarction, ECG change rate of brainstem lesions was significantly higher than basal ganglia, thalamic lesions and brain lobes, cortical lesions(P <0.05), ECG change rate of basal ganglia, thalamic lesions was higher than brain lobes, cortical lesions. HF, LF, TF, SDNN, RMSSD and LVEF of observation group were lower than control group significantly(P <0.05). 4 patients with ECG changes died, prognosis in patients with ECG changes was poor than patients without ECG changes, HRV and LVEF was lower in patients with ECG changes. Conclusion The closer of Infarction focus and cardiovascular center, the more ECG changes, HRV and LVEF decreases with cerebral infarction exacerbation.
