目的:通过对类风湿关节炎(RA)活动性指标血清白介素-2(IL-2)、透明质酸(HA)、血管渗透因子(VEGF)水平变化的测定,探讨它们与病情活动的相关性及临床意义。方法采用 RIA 和 ELISA 法对类风湿关节炎患者活动组、缓解组及正常对照组进行 IL-2、HA、VEGF 测定,并从敏感性、特异性方面进行比较分析。结果活动组 RA 患者血清HA、VEGF 的含量显著高于缓解组及正常对照组(P<0.01),而活动组 RA 患者血清 IL-2的含量却显著低于缓解组及正常对照组(P<0.01)。VEGF 的特异性最强(93.3%),敏感性为93.3%。结论与血清 IL-2、HA 相比,血清 VEGF 水平测定更有助于类风湿关节炎活动性的监测,对该病的治疗及预后均有一定的意义。

목적:통과대류풍습관절염(RA)활동성지표혈청백개소-2(IL-2)、투명질산(HA)、혈관삼투인자(VEGF)수평변화적측정,탐토타문여병정활동적상관성급림상의의。방법채용 RIA 화 ELISA 법대류풍습관절염환자활동조、완해조급정상대조조진행 IL-2、HA、VEGF 측정,병종민감성、특이성방면진행비교분석。결과활동조 RA 환자혈청HA、VEGF 적함량현저고우완해조급정상대조조(P<0.01),이활동조 RA 환자혈청 IL-2적함량각현저저우완해조급정상대조조(P<0.01)。VEGF 적특이성최강(93.3%),민감성위93.3%。결론여혈청 IL-2、HA 상비,혈청 VEGF 수평측정경유조우류풍습관절염활동성적감측,대해병적치료급예후균유일정적의의。
Objective To explore the relationship between rheumatoid activity and serum levels of the cytokines interleukin-2 (IL-2),hyaluronic acid (HA), vascular endothelial growth factor (VEGF) in patients with rheumatoid arthritis. Methods Serum IL-2, HA (with RIA) and VEGF (with ELISA) levels were determined in 30 controls, 30 patients with active rheumatoid arthritis and 30 patients with rheumatoid arthritis in remission. Sensitivity and spectificity for each marker were analyzed.Results In patients with active rheumatoid arthritis, the serum levels of HA, VEGF were significantly higher and serum levels of IL-2 significantly lower than those in patients in remission and controls (P<0.01).Determination of VEGF levels possessed the highest specificity (93.3%) and also a high sensitivity (93.3% as well).Conclusion Determination of the serum levels of any of these markers was valuable for monitoring the activity of the rheumatoid process. It is more desirable to take measarements of VEGF levels due to its highest specificity.
