目的:检测人肺癌组织中的Hedgehog信号通路主要成分SHH、Gli-1和血管内皮生长因子( VEGF)的表达,探讨其与不同临床病理特征的相关性。方法选取2013年3-10月广西医科大学第一附属医院临床病理明确诊断为肺癌,并行外科手术治疗的患者72例,取癌组织及癌旁组织(距离癌组织5 cm),另选取同时期良性肺病变组织30例。收集肺癌患者临床病理资料,免疫组织化学法检测SHH、Gli-1和VEGF的表达,实时荧光定量PCR检测其mRNA的表达。结果肺癌组织中SHH阳性表达率为73.6%(53/72),癌旁组织为16.7%(12/72),良性肺病变组织为6.7%(2/30),差异有统计学意义(χ2=64.819,P<0.05)。肺癌组织中Gli-1阳性表达率为30.6%(22/72),癌旁组织为8.3%(6/72),良性肺病变组织为6.7%(2/30),差异有统计学意义(χ2=15.300,P<0.05)。肺癌组织中VEGF阳性表达率为75.0%(54/72),癌旁组织为69.4%(50/72),良性肺病变组织为16.7%(5/30),差异有统计学意义(χ2=34.530,P<0.05)。不同性别、年龄、吸烟史、分化程度、术前放化疗肺癌患者Gli-1阳性表达率比较,差异均有统计学意义( P<0.05);不同淋巴结转移、远处转移、TNM分期、早晚期、分化程度、病理类型、术前放化疗肺癌患者VEGF阳性表达率比较,差异均有统计学意义( P<0.05)。肺癌组织中SHH mRNA相对表达量为(60.857±5.446),癌旁组织为(10.848±1.600),良性肺病变组织为(25.763±2.178),差异有统计学意义(F=110.982,P<0.05)。肺癌组织中Gli-1 mRNA相对表达量为(9.804±1.054),癌旁组织为(5.661±0.612),良性肺病变组织为(3.551±0.545),差异有统计学意义(F=59.183,P<0.05)。肺癌组织中VEGF mRNA相对表达量为(0.574±0.051),癌旁组织为(0.495±0.027),良性肺病变组织为(0.357±0.030),差异有统计学意义(F =6.061,P<0.05)。不同性别、吸烟史、TNM分期、分化程度、病理类型、术前放化疗肺癌患者Gli-1 mRNA相对表达量比较,差异均有统计学意义( P<0.05);不同分化程度、术前放化疗肺癌患者VEGF mRNA相对表达量比较,差异均有统计学意义( P <0.05)。肺癌组织中 SHH、Gli -1和 VEGF的表达与 mRNA 相对表达量均呈正相关( P <0.05)。结论 Hedgehog信号通路主要成分SHH、Gli-1及VEGF在肺癌中高表达,与肿瘤的发展密切相关;联合抑制Hedgehog信号通路及VEGF是肺癌分子靶向治疗的有效措施。

Objective To study the expression of Hedgehog signaling pathway molecules SHH,Gli-1 and VEGF in human lung cancer,then explore the relationship between these biomarkers and clinicopathologic characteristics. Methods From March 2013 to October 2013 in the First Affiliated Hospital of Guangxi Medical University,72 cases with pathologically confirmed lung cancer after surgical treatment were enrolled,chosing cancer tissues and cancer adjacent tissues( over 5 cm from cancer tis-sue) in every case. As control,30 cases of normal lung tissues were selected. Immunochemical stain( IHC)method was em-ployed to detect the expression of SHH,Gli-1 and VEGF in the samples,RT-PCT was used to assess mRNA levels of SHH,Gli-1 and VEGF. Results The positive rate of SHH were 73. 6%(53/72) in cancer tissues,16. 7%(12/72) in cancer ad-jacent tissues and 6. 7%( 2/30 ) in control. Similar tendency was also found in the other 2 biomarkers, the positive rate of Gli-1 were 30. 6%(22/72) in cancer tissues,8. 3%(6/72) in cancer adjacent tissues and 6. 7%(2/30) in control. The positive rate of VEGF were 75. 0%(54/72)in cancer tissues,69. 4%(50/72)in cancer adjacent tissues and 16. 7%(5/30) in control. Expression of SHH,Gli-1 and VEGF were significantly higher in cancer tissues compared with normal control ( P<0. 05),and the score ofχ2 were 64. 819,15. 300 and 34. 530 for SHH,Gli-1 and VEGF. There were significant correla-tions between Gli-1 expression and following clinicopathologic characteristics:gender,age,smoking status,degrees of differ-entiation,preoperative chemotherapy(P<0. 05). As to VEGF expression,there were significant correlations between it and following clinicopathologic characteristics:lymphatic metastasis,distant metastasis,TNM stage,disease stage,degrees of dif-ferentiation,histology,preoperative chemotherapy(P<0. 05). Comparing with normal control,mRNA levels of SHH,Gli-1 and VEGF genes expressed significantly higher in cancer tissue(P<0. 05). The relative quantity of SHH mRNA were(60. 857 ±5. 446)in cancer tissues,(10. 848 ±1. 600)in cancer adjacent tissues,(25. 763 ±2. 178)in control. The relative quantity of Gil-1 mRNA were(9. 804 ±1. 054)in cancer tissues,(5. 661 ±0. 612)in cancer adjacent tissues,(3. 551 ±0. 545)in control. The relative quantity of VEGF mRNA were(0. 574 ± 0. 051) in cancer tissues,(0. 495 ± 0. 027)in cancer adjacent tis-sues,( 0. 357 ± 0. 030 ) in control. There were significant correlations between Gli -1 mRNA and following clinicopathologic characteristics:gender,smoking status,TNM stage,degrees of differentiation,histology,preoperative chemotherapy( P <0. 05). As to VEGF mRNA,there were significant correlations between it and following clinicopathologic characteristics:de-grees of differentiation,preoperative chemotherapy(P<0. 05). Expression of SHH,Gli-1 and VEGF were positive correlated with mRNA level of SHH,Gli-1 and VEGF(P<0. 05). Conclusion Expression of Hedgehog signaling pathway molecules SHH,Gli-1 and VEGF were significantly higher in lung cancer,which were closely related to the development of lung cancer and its clinicopathologic characteristics. Hedgehog signaling pathway molecules and VEGF might be a potential therapeutic targetin lung cancer treatment.
