目的本研究拟通过QRS波时限指导找到理想的右室间隔起搏部位。方法选取40例行心内电生理检查的患者,将标准大头导管分别置于同一患者的右室间隔低位、中位、高位三个位点起搏,以获得最窄QRS波时限的右室间隔区域作为最佳右室间隔起搏部位。结果右室间隔起搏QRS波时限,中位组明显窄于高位或低位组(136.7±16.1 ms vs 154.9±16.7ms,149.8±18.0ms)(<0.001)。结论右室中位间隔面起搏后QRS波时限较窄,为最佳右室间隔起搏部位,这将为临床选择更合理的起搏部位提供实验依据。

목적본연구의통과QRS파시한지도조도이상적우실간격기박부위。방법선취40례행심내전생리검사적환자,장표준대두도관분별치우동일환자적우실간격저위、중위、고위삼개위점기박,이획득최착QRS파시한적우실간격구역작위최가우실간격기박부위。결과우실간격기박QRS파시한,중위조명현착우고위혹저위조(136.7±16.1 ms vs 154.9±16.7ms,149.8±18.0ms)(<0.001)。결론우실중위간격면기박후QRS파시한교착,위최가우실간격기박부위,저장위림상선택경합리적기박부위제공실험의거。
Objective This study was designed to explore the optimal position of right ventricular septum(RVS)pacing by assessing QRS wave duration. Methods Pacing and electrocardiography recording were underwent, right ventricular septum (RVS)were divided into 3 pans respectively (high,middle and low),40 patients who accept electrophysiological examination. Then the duration and morphology of QRS complex wave paced in dif erent right ventricular locations were analysed and compared. The best stimulation place was the position which results the narrowest QRS wave durations. Results The width of post-pacing QRS wave was narrowest at middle septal (136.7±16.1ms),had significant dif erence compared with high septal(154.9±16.7)ms or the low septal(149.8±18.0)ms ( <0.001). Conclusion The width of post-pacing QRS wave in the middle septum group is narrowest,it maybe the optimal position of RVS pacing.
