目的：分析不明原因发热(fever of unknown origin，FUO)中上呼吸道感染(upper respiratory tract infection，URTI)的临床特点，提高临床诊断水平。方法回顾性分析本院发热疾病科2009年2月-2012年12月间符合FUO诊断标准的URTI患者的临床资料，分为男性组和女性组，年龄＜40岁组和≥40岁组进行比较。结果本研究共纳入URTI 58例，平均年龄38.6岁，平均发热时间25.1 d，75.9%表现为间歇热，男女比较差异无统计学意义，年龄≥40岁组较年龄＜40岁组确诊时间长(P=0.0077)、医疗花费多(P=0.0000)。结论表现为长期发热的上呼吸道感染患者症状体征不典型，多可自愈。如能早期诊断可减少患者的恐慌及医疗资源的浪费。

Objective To improve the clinical diagnosis of upper respiratory tract infection (URTI) in patients with fever of unknown origin (FUO) by analyzing its clinical features.Methods The clinical data about URTI patients with FUO admitted to our department from February 2009 to December 2012 were retrospectively analyzed. The patients were divided into groups of sex(male and female) and age (<40 years and≥40 years). Their clinical data were compared.Results Fifty-eight URTI patients were enrolled in this study with a mean age of 38.6 years and a mean fever time of 25.1 days. URTI was manifested as intermittent fever in 75.9% of the patients. No significant difference was found in gender. More time and expense of diagnosis were observed in age≥40 year's group (P=0.007 7,P=0.000 0).Conclusion The symptoms and signs of URTI which is manifested as chronic FUO are atypical and most of them are selflimited. The fear and medical cost of such patients can be reduced if it is early diagnosed.
