目的：探讨首发精神分裂症患者体内阿立哌唑血药浓度与临床应用的关系。方法采用高效液相色谱(HPLC)的方法测定60例首发精神分裂症患者体内阿立哌唑的血浆药物浓度，并用阳性与阴性症状量表(PANSS)评定疗效，治疗副反应量表(TESS)评定不良反应，并对血药浓度与疗效评分进行相关性分析，结合受试者特征曲线（ROC曲线）推测阿立哌唑最佳血药浓度。结果阿立哌唑血药浓度与药物剂量、PANSS减分率及TESS分值呈正相关。最低有效血药浓度为364μg/L，发生不良反应的血药浓度阈值为550μg/L。血药浓度在364-550μg/L的范围内疗效较好，副反应较轻。结论阿立哌唑治疗精神分裂症的最佳血药浓度为364-550μg/L，血药浓度和临床疗效存在一定的相关性。
목적：탐토수발정신분렬증환자체내아립고서혈약농도여림상응용적관계。방법채용고효액상색보(HPLC)적방법측정60례수발정신분렬증환자체내아립고서적혈장약물농도，병용양성여음성증상량표(PANSS)평정료효，치료부반응량표(TESS)평정불량반응，병대혈약농도여료효평분진행상관성분석，결합수시자특정곡선（ROC곡선）추측아립고서최가혈약농도。결과아립고서혈약농도여약물제량、PANSS감분솔급TESS분치정정상관。최저유효혈약농도위364μg/L，발생불량반응적혈약농도역치위550μg/L。혈약농도재364-550μg/L적범위내료효교호，부반응교경。결론아립고서치료정신분렬증적최가혈약농도위364-550μg/L，혈약농도화림상료효존재일정적상관성。
Objective To explore the relationship between serum concentration and clinical efficacy of Aripiprazole in the treatment of first episode schizophrenia. Methods The trough serum concentrations were determined by high performance liquid chromatography (HPLC). The clinical efficacy was evaluated with the positive and negative syndrome scale (PANSS) and side ef-fects were done with the treatment emergent symptom Scale (TESS). The correlation among serum concentration and dosage ,thera-peutic effect and side effect of Aripiprazole were analyzed,and the optimized cut-off value of serum concentration was procured by the receiver operator curve (ROC). Results The trough serum concentration was significantly positive correlated with dosages ,the decreasing ratios of PANSS scores and TESS scores respectively. The lowest effective trough serum concentration was 364μg/L,the trough serum concentration threshold occurring side effects was 550μg/L. The efficacy of Aripiprazole was best and its side effects were mildest when serum Aripiprazole levels were in the range of 364-550μg/L. Conclusion The serum concentration of aripipra-zole in the treatment of patients with schizophrenia is correlated with clinical efficacy. The suitable therapeutic window for patients treated with Aripiprazole is 364-550μg/L.
