目的分析肝结核CT动态增强的CT表现,探讨CT动态增强在肝结核诊断和鉴别诊断。方法回顾性分析11例肝结核CT动态增强表现。结果11例中粟粒性肝结核1例,局限性肝结核5例,结核瘤型肝结核4例,肝包膜结核1例。粟粒性肝结核:(1)肝密度偏低不均匀,(2)增强在正常肝组织对比下可见弥漫性大小不一粟粒状、小结节状低密度灶;局限性肝结核:(1)平扫单发或多发结节状、片状低密度灶,(2)增强病灶各期边缘轻到中度强化,(3)中心不均匀轻度强化或无强化;结核瘤型肝结核:(1)平扫呈结节状低密度灶,(2)增强动脉期无或轻度周围强化,(3)门脉期病灶周围环形轻到中度强化(4)均有延迟强化,(5)病灶中心各期均无强化。结论 CT动态增强对肝结核诊断有较高诊断价值,结合临床和肝外结核病史多可以做出诊断。
目的分析肝結覈CT動態增彊的CT錶現,探討CT動態增彊在肝結覈診斷和鑒彆診斷。方法迴顧性分析11例肝結覈CT動態增彊錶現。結果11例中粟粒性肝結覈1例,跼限性肝結覈5例,結覈瘤型肝結覈4例,肝包膜結覈1例。粟粒性肝結覈:(1)肝密度偏低不均勻,(2)增彊在正常肝組織對比下可見瀰漫性大小不一粟粒狀、小結節狀低密度竈;跼限性肝結覈:(1)平掃單髮或多髮結節狀、片狀低密度竈,(2)增彊病竈各期邊緣輕到中度彊化,(3)中心不均勻輕度彊化或無彊化;結覈瘤型肝結覈:(1)平掃呈結節狀低密度竈,(2)增彊動脈期無或輕度週圍彊化,(3)門脈期病竈週圍環形輕到中度彊化(4)均有延遲彊化,(5)病竈中心各期均無彊化。結論 CT動態增彊對肝結覈診斷有較高診斷價值,結閤臨床和肝外結覈病史多可以做齣診斷。
목적분석간결핵CT동태증강적CT표현,탐토CT동태증강재간결핵진단화감별진단。방법회고성분석11례간결핵CT동태증강표현。결과11례중속립성간결핵1례,국한성간결핵5례,결핵류형간결핵4례,간포막결핵1례。속립성간결핵:(1)간밀도편저불균균,(2)증강재정상간조직대비하가견미만성대소불일속립상、소결절상저밀도조;국한성간결핵:(1)평소단발혹다발결절상、편상저밀도조,(2)증강병조각기변연경도중도강화,(3)중심불균균경도강화혹무강화;결핵류형간결핵:(1)평소정결절상저밀도조,(2)증강동맥기무혹경도주위강화,(3)문맥기병조주위배형경도중도강화(4)균유연지강화,(5)병조중심각기균무강화。결론 CT동태증강대간결핵진단유교고진단개치,결합림상화간외결핵병사다가이주출진단。
ObjectiveTo evaluate the diagnostic value of dynamic contrast-enhanced CT in hepatic tuberculosis(TB).Methods CT of 11 patients with hepatic TB was analyzed retrospectively.Results Of 11 patients, the TB was miliary(1), localized(5), tuberculoma(4) or capsular(1). Miliary hepatic TB showed heterogeneous low density or diffuse hypodense micronodules of varying size. Localized TB appeared as solitary or multiple nodular and flaky hypodense foci with mild to moderate border enhancement in all phases, mild or no enhancement in the center. Tuberculomas were hypodense nodules with mild or no rim enhancement in the arterial phase, mild to moderate enhancement in the venous phase, persistent delayed enhancement, and no enhancement in the center.Conclusion Dynamic contrast-enhanced CT can aid diagnosis of hepatic TB combined with clinical history.
