目的:评价三维动态增强磁共振血管成像(3D DCE-MRA)显示头颈部血管情况并与数字减影血管造影( DSA)结果对照,探讨3D DCE-MRA在头颈部动脉狭窄的临床应用价值。方法:有头颈部血管狭窄病变的50例病人进行3D DCE-MRA检查,采用K空间中心优先采集技术,3D FLASH序列进行扫描,扫描范围自颅顶至主动脉弓。获得满意的头颈部任意位置任意角度的三维动脉血管像。结果:与DSA比较3D DCEMRA诊断头颈部血管狭窄>50%的敏感性和特异性分别为97.88%和97.83%,与DSA比较有较好一致性,但3D DCEMRA存在过高估计血管狭窄程度。结论:三维动态增强磁共振血管成像(3D DCE-MRA)是一种无创、无辐射、便宜的血管成像方法。

목적:평개삼유동태증강자공진혈관성상(3D DCE-MRA)현시두경부혈관정황병여수자감영혈관조영( DSA)결과대조,탐토3D DCE-MRA재두경부동맥협착적림상응용개치。방법:유두경부혈관협착병변적50례병인진행3D DCE-MRA검사,채용K공간중심우선채집기술,3D FLASH서렬진행소묘,소묘범위자로정지주동맥궁。획득만의적두경부임의위치임의각도적삼유동맥혈관상。결과:여DSA비교3D DCEMRA진단두경부혈관협착>50%적민감성화특이성분별위97.88%화97.83%,여DSA비교유교호일치성,단3D DCEMRA존재과고고계혈관협착정도。결론:삼유동태증강자공진혈관성상(3D DCE-MRA)시일충무창、무복사、편의적혈관성상방법。
Objective:To evaluate 3D dynamic contrast enhanced magnetic resonance angiography (3D DCE-MRA )show blood ves-sels of head and neck,and compare it with digital subtraction angiography (DSA)and discuss the value of clinical application of 3D DCEMRA on vascular disease of head and neck .Methods:Fifty patients suspected of having blood vessel diseases under went 3D DCE-MRA,K-space center priority collection technology and 3D FLASH sequence was performed .The satisfying 3D arterial image of head and neck in any position or arbitrary angle was obtained through 3D MIP resconstrction of the subtraction images .Results:3DCEMRA and DSA, comparing the evaluation of head and neck hemal stricture degree .Compared with DSA, the sensitivity and specificity for detection of significant stricture (>50%) were 97.88%and 97.83%.Conclusion:Compared with DSA, 3D DCE-MRA might be a method of noninvasive , no radiation , cheap in the evaluation of the neck and proximal intracranial vascular .
