目的:观察绝对不应期电刺激即心脏收缩力调节信号(CCM)对兔缺血性心力衰竭模型心功能的影响。方法30只新西兰大白兔采用结扎冠状动脉前降支的方法制备缺血性心力衰竭动物模型。造模成功的实验动物随机分为心力衰竭组对照组、左心室侧壁刺激组、左心室后壁刺激组。刺激组发放CCM信号,每天刺激6 h,连续刺激7 d。观察心脏功能的变化。结果与心力衰竭对照组相比,左心室侧壁刺激组及左心室后壁刺激组的左心室收缩末内径、左心室舒张末内径、血浆B型尿钠肽(BNP)水平、心脏重量、心脏重量指数下降(P<0.05)。左心室射血分数和左心室短轴缩短率升高(P<0.05)。QTcd各组之间相比未见明显差异(P>0.05)。结论绝对不应期电刺激能够增强缺血性心力衰竭兔心肌收缩力,逆转心脏重塑,改善心功能,且并不增加恶性心律失常发生的风险。

Objective To investigate the effects of electric stimulations applied during absolute refractory period on cardiac function in rabbits with ischemic heart failure. Methods Heart failure animal model was established by blocking left anterior descending coronary artery. Those animal models, which are fitted standard, were randomly divided into three groups: Heart failure control group, Left ventricular lateral wall cardiac contractility modulation group(LVLW-CCM), Left ventricular posterior wall cardiac contractility modulation group(LVPW-CCM). Then electrical stimulations during the absolute refractory period were delivered to each group lasting six hours everyday for seven days. Changes in cardiac function were observed after CCM stimulation. Results Compared with Heart failure control group, LVESD, LVEDD, plasma BNP levels, heart weight, and heart weight index in LVLW-CCM and LVPW-CCM group were significantly decreased (P<0.05), while their LVEF and FS were significantly increased (P<0.05);QTcd had no statistical difference among the three groups after CCM. Conclusion Electric stimulations applied during absolute refractory period can enhance the contractility of myocardium, reverse ventricular remodeling and improve cardiac function, while it has no influence on the incidence of malignant arrhythmia.
