目的：探讨64层螺旋CT肺动脉CTA对诊断肺动脉血栓的应用价值。方法对接受检查的肺动脉栓塞患者（50例），使用64层螺旋CT肺动脉CTA对患者进行诊断后再进行病理探查，采用不同的CTA方法对患者进行检查，分析影响因素，然后选择最佳肺动脉CTA检查方法对患者进行诊断，计算灵敏度及特异度。结果单因素分析发现扫描方向、对比剂用量对诊断结果的影响差异有统计学意义（P＜0．05），回归分析发现，足-头侧扫描及对比剂用量90 mL的影响差异具有统计学意义（P＜0．05）。采用最佳的CTA诊断方法对患者进行诊断后发现，肺动脉CTA诊断的灵敏度98．0％，特异度96．0％，与手术病例结果比较差异有统计学意义（P＞0．05）。结论肺动脉CTA对肺动脉血栓的诊断灵敏度和特异度高，扫描方式及对比剂用量可影响诊断结果。
목적：탐토64층라선CT폐동맥CTA대진단폐동맥혈전적응용개치。방법대접수검사적폐동맥전새환자（50례），사용64층라선CT폐동맥CTA대환자진행진단후재진행병리탐사，채용불동적CTA방법대환자진행검사，분석영향인소，연후선택최가폐동맥CTA검사방법대환자진행진단，계산령민도급특이도。결과단인소분석발현소묘방향、대비제용량대진단결과적영향차이유통계학의의（P＜0．05），회귀분석발현，족-두측소묘급대비제용량90 mL적영향차이구유통계학의의（P＜0．05）。채용최가적CTA진단방법대환자진행진단후발현，폐동맥CTA진단적령민도98．0％，특이도96．0％，여수술병례결과비교차이유통계학의의（P＞0．05）。결론폐동맥CTA대폐동맥혈전적진단령민도화특이도고，소묘방식급대비제용량가영향진단결과。
Objective To investigate the application value of 64-slice spiral CT pulmonary CTA for the diagnosis of pulmonary thromboembolism .Methods For 50 patients with pulmonary embolism who underwent radiological examination from September 2011 to Sep-tember 2012 in our hospital ,64-slice spiral CT pulmonary CTA was used for pathological diagnosis during exploration .Different methods were applied for patients with CTA inspection ,the influencing factors were analysed ,and then the best method was chosen to diagnose patients with pulmonary CTA ,finally the sensitivity and specificity were calculated .Results Univariate analysis showed that the scan direction ,contrast a-gent dosage showed significant difference in the diagnosis ( P<0.05);regression analysis demonstrated that foot-cephalic scan and contrast dosage 90mL impact had significant difference (P<0.05 ).Usage of the best CTA diagnostic method found that the diagnostic sensitivity of pulmonary CTA was 98.0%,and the specificity was 96.0%,which had no significant difference compared with the pathological examination (P>0.05).Conclusion Pulmonary CTA for the diagnosis of pulmonary thromboembolism has high sensitivity and specificity .The diagnos-tic results are mainly influenced by scanning and contrast dosage .Reasonable adjustments of diagnostic mode can be made according to pa-tients'clinical condition .
