目的:探讨青年人膝关节运动性损伤的MRI特点,并与临床功能评分对照。材料与方法回顾性分析30例X线阴性的膝关节的MRI资料,由两位放射科医师协商分析其信号特点,并行MRI评分。临床功能评分则由另一位骨科医师完成。两种评分方法间行相关性分析(Pearson),当P<0.05认为有统计学差异。结果30例膝关中,73.33%(22/30)为单发骨挫伤病灶;26.67%(8/30)为两或多发骨挫伤病灶,共计50处。骨挫伤病灶表现为骨皮质下髓质区的长或稍长T1长T2信号,形态呈地图样、片状、斑点状等。63.33%(19/30)合并半月板损伤,63.33%(19/30)韧带损伤,96.67%(29/30)关节积液,50%(15/30)关节周围软组织损伤。膝关节MRI评分为5.00±1.95,膝关节功能评分为5.03±1.73,两者的相关性高(r=0.89, P=0.00)。结论 MRI能清楚地显示运动性膝关节的损伤部位及形态,并可以对其严重程度定量评分,且与临床功能评分的相关性好。

목적:탐토청년인슬관절운동성손상적MRI특점,병여림상공능평분대조。재료여방법회고성분석30례X선음성적슬관절적MRI자료,유량위방사과의사협상분석기신호특점,병행MRI평분。림상공능평분칙유령일위골과의사완성。량충평분방법간행상관성분석(Pearson),당P<0.05인위유통계학차이。결과30례슬관중,73.33%(22/30)위단발골좌상병조;26.67%(8/30)위량혹다발골좌상병조,공계50처。골좌상병조표현위골피질하수질구적장혹초장T1장T2신호,형태정지도양、편상、반점상등。63.33%(19/30)합병반월판손상,63.33%(19/30)인대손상,96.67%(29/30)관절적액,50%(15/30)관절주위연조직손상。슬관절MRI평분위5.00±1.95,슬관절공능평분위5.03±1.73,량자적상관성고(r=0.89, P=0.00)。결론 MRI능청초지현시운동성슬관절적손상부위급형태,병가이대기엄중정도정량평분,차여림상공능평분적상관성호。
Objective To investigate the MRI characteristic of sporting related knee joint injury of youth, and compare with clinical functional score. MethodsandMaterial The MRI imaging data of 30 cases were analyzed retrospectively in this study while the results of X-ray were negative. The MRI findings of the knee joints were summarized and scored by two radiologic doctor, and the functional of the knee joints was scored by an orthopaedic doctor. The Pearson was employed to analyze correlation between two methods used for quantitative score, and there is a significant difference when P<0.05. Results In 30 cases of knee joints, 73.33%(22/30) shown single lesion of osteal contusion and 26.67%(8/30) cases present with two or more lesions, a total of 50 lesions. The osteal contusion appear low or slightly low signal on T1WI and high signal intensity on T2WI, and appear as geographic, patchy and mottling shapes. Osteal contusion of knee joints were accompanied by 63.33% (19/30) meniscus injury and ligament injury respectively, 96.67% (29/30) joint effusion, 50% (15/30) joints surrounding soft tissue injury. The mean MRI score is 5.00±1.95, functional score of 5.03±1.73, and the a good correlation between two grading methods (r=0.89, P=0.00). Conclusion MRI imaging can detect the location and shapes of sporting related osteal contusion in knee joints, and can grade quantitatively the injury severity. There is also a good correlation between MR score and functional score used to assess the injury severity of sporting related knee joints.
