目的:分析总结中枢神经细胞瘤(CNC)的CT及MRI影像学特征,以对其进行正确诊断。方法对23例经手术病理证实的中枢神经细胞瘤的CT及MRI影像学表现进行回顾性分析,总结其影像特征。结果23例中枢神经细胞瘤多位于侧脑室前部、孟氏孔区,向侧脑室两侧不对称性生长,可见分叶,边界清晰。CT多呈等或稍高密度阴影,其中可见囊变及钙化。MRI表现为肿瘤瘤体信号不均匀, T1WI呈等、稍低或稍高信号,T2WI呈等或稍高信号,肿块可见囊变,钙化或出血,有14例可见肿瘤内的多发性囊肿与侧脑室壁相连。CT和MRI增强扫描见病灶实性部分大多为轻中度强化,囊变和钙化区无强化。结论中枢神经细胞瘤的CT和MRI表现具备一定的特征性,CNC的MR重要征象之一为肿瘤的实性部分和侧脑室壁之间有多发性囊肿相连。

목적:분석총결중추신경세포류(CNC)적CT급MRI영상학특정,이대기진행정학진단。방법대23례경수술병리증실적중추신경세포류적CT급MRI영상학표현진행회고성분석,총결기영상특정。결과23례중추신경세포류다위우측뇌실전부、맹씨공구,향측뇌실량측불대칭성생장,가견분협,변계청석。CT다정등혹초고밀도음영,기중가견낭변급개화。MRI표현위종류류체신호불균균, T1WI정등、초저혹초고신호,T2WI정등혹초고신호,종괴가견낭변,개화혹출혈,유14례가견종류내적다발성낭종여측뇌실벽상련。CT화MRI증강소묘견병조실성부분대다위경중도강화,낭변화개화구무강화。결론중추신경세포류적CT화MRI표현구비일정적특정성,CNC적MR중요정상지일위종류적실성부분화측뇌실벽지간유다발성낭종상련。
Objective To assess the value of CT and MRI in the diagnosis of central neuroeytoma. Methods CT and MRI findings of 23 cases with central neurocytoma verified by pathology were analyzed retrospectively. Results 23 cases of central neurocytoma were Iocated in the frontal of lateral ventricle adjacent to foramen of Monro. Most of them were irregular in shape, asymmetrically grown to both sides, clearly bounded and lobulated, there was no or only slight edema in brain. The tumor showed iso-or high density on CT imaging. All tumours had heterogeneous signal intensity in their solid components. The lesions demonstrated iso-, slightly hypo-or hyperintense on Tl-weighted image and iso-or slightly hyperintense on T2-weighted image. The tumor showed cystic change and calcification or bleeding in the mass.Fourteen could be seen multiple cysts interfacing between the solid part of the tumour and the lateral ventricular wall. The solid part could be enhanced slightly or moderatedly and the part of cystic change and calcification could not be enhanced on CT and MR images. Conclusion Central neurocytoma have some characteristics on CT and MR images, multiple cystic interfaces between the solid part of the tumour and the lateral ventricular wall on MRI may contribute to a correct preoperative diagnosis of CNC.
