目的:对比分析CT和MRI对于颈静脉孔区原发肿瘤病变的诊断价值。方法回顾性分析20例经手术病例证实为颈静脉孔区肿瘤病变的CT和MRI表现。结果20例患者中脑膜瘤3例,颈静脉球瘤8例,神经鞘瘤8例,高分化软骨肉瘤1例。CT和MRI均清晰显示颈静脉孔增大,其中CT显示骨质吸收破坏11例、骨质浸蚀破坏9例,而MRI误判4例骨质浸蚀破坏为吸收破坏。MRI及CT显示神经鞘瘤纵向分布体征分别为8例、4例,显示神经鞘瘤囊变分别为5例、2例;MRI显示颈静脉球瘤“椒盐”征6例,而CT仅显示瘤内迂曲增粗血管2例;MRI显示脑膜瘤“脑膜尾”征3例,而CT显示不清;CT清晰显示软骨肉瘤钙化特征。结论 CT及MRI对于颈静脉孔区原发肿瘤具有重要诊断价值,MRI对于瘤体整体分布、细节观察优势明显,而CT更易显示病灶周围骨质改变及病灶内钙化情况。
目的:對比分析CT和MRI對于頸靜脈孔區原髮腫瘤病變的診斷價值。方法迴顧性分析20例經手術病例證實為頸靜脈孔區腫瘤病變的CT和MRI錶現。結果20例患者中腦膜瘤3例,頸靜脈毬瘤8例,神經鞘瘤8例,高分化軟骨肉瘤1例。CT和MRI均清晰顯示頸靜脈孔增大,其中CT顯示骨質吸收破壞11例、骨質浸蝕破壞9例,而MRI誤判4例骨質浸蝕破壞為吸收破壞。MRI及CT顯示神經鞘瘤縱嚮分佈體徵分彆為8例、4例,顯示神經鞘瘤囊變分彆為5例、2例;MRI顯示頸靜脈毬瘤“椒鹽”徵6例,而CT僅顯示瘤內迂麯增粗血管2例;MRI顯示腦膜瘤“腦膜尾”徵3例,而CT顯示不清;CT清晰顯示軟骨肉瘤鈣化特徵。結論 CT及MRI對于頸靜脈孔區原髮腫瘤具有重要診斷價值,MRI對于瘤體整體分佈、細節觀察優勢明顯,而CT更易顯示病竈週圍骨質改變及病竈內鈣化情況。

Objective To investigate the value of MRI and CT in the diagnosis of primary tumor in jugular foramen area. Methods CT and MRI findings in 20 cases of primary tumour in jugular foramen area,which were proved by surgery and pathology were retrospectively analysed. Results Of 20 cases, meningiomas(n=3), glomus jugular tumors (n=8), jugular foramen schwannomas (n=8) and well differentiated chondrosarcoma (n=1) were detected. The jugular foramen enlargement could be showed both in CT and MRI in 20 cases. 11 cases had bone absorption in cases and bone erosion in 9 cases could be showed in CT,but MRI mistook bone erosion for bone absorption in 4 cases. For schwannomas, the characteristic of longitudinal distribution was pictured by MRI in 8 cases and by CT in 4 cases, cystic changes were revealed by MRI in 5 cases and by CT in 2 cases. The"salt sand pepper"sign could be displayed in MRI in 6 cases of glomus jugular tumors ,but only the tortuous and dialted blood vessels only could be showed in enhanced CT in 2 cases of glomus jugular tumors. The"tail sign"could be clearly displayed in MRI in 3 cases of meningiomases, but CT displayed obscurely. The character of calcification could be showed CT in chondrosarcoma. Conclusion Both CT and MRI were important in diagosing primary tumour in jugular foramen area. MRI was expert to display the distrubution and details,CT was good at displaying bone change and calcification .
