目的：探讨经脐血干细胞移植治疗失代偿期肝硬化患者外周血白细胞计数、分类以及凝血功能的变化及其临床意义。方法选取50例失代偿期肝硬化患者，经肝动脉灌注移植脐血干细胞治疗，回顾性分析移植前后白细胞总数、中性粒细胞百分比、淋巴细胞百分比以及凝血功能的变化。结果8例已行脾切除术的肝硬化患者外周血白细胞总数及血小板水平均在正常范围；42例肝硬化患者在脐血干细胞移植术前白细胞总数、血小板计数低于正常水平，经脐血干细胞治疗后外周血白细胞数量及血小板数量在总体上呈上升趋势，中性粒细胞百分比呈下降趋势，淋巴细胞百分比则呈上升趋势；术后第16周白细胞总数为（4．01±1．02）×109/L，中性粒细胞百分比为（58．7±7．2）％，淋巴细胞百分比为（35．8±6．3）％，与术前相比差异有统计学意义（P＜0．05）；50例肝硬化患者在脐血干细胞移植后凝血功能均有明显改善，差异有统计学意义（P＜0．05）。结论脐血干细胞移植可以改善肝硬化患者的凝血功能，提升白细胞总数、淋巴细胞百分比及降低中性粒细胞百分比。
목적：탐토경제혈간세포이식치료실대상기간경화환자외주혈백세포계수、분류이급응혈공능적변화급기림상의의。방법선취50례실대상기간경화환자，경간동맥관주이식제혈간세포치료，회고성분석이식전후백세포총수、중성립세포백분비、림파세포백분비이급응혈공능적변화。결과8례이행비절제술적간경화환자외주혈백세포총수급혈소판수평균재정상범위；42례간경화환자재제혈간세포이식술전백세포총수、혈소판계수저우정상수평，경제혈간세포치료후외주혈백세포수량급혈소판수량재총체상정상승추세，중성립세포백분비정하강추세，림파세포백분비칙정상승추세；술후제16주백세포총수위（4．01±1．02）×109/L，중성립세포백분비위（58．7±7．2）％，림파세포백분비위（35．8±6．3）％，여술전상비차이유통계학의의（P＜0．05）；50례간경화환자재제혈간세포이식후응혈공능균유명현개선，차이유통계학의의（P＜0．05）。결론제혈간세포이식가이개선간경화환자적응혈공능，제승백세포총수、림파세포백분비급강저중성립세포백분비。
Objective to explore the effect of umbilical cord blood stem cell transplantation in pa-tients with decompensated cirrhosis on peripheral blood leukocyte count ,classification and coagulation function and its clinical significance .Methods Select 50 cases of patients with decompensated cirrhosis , umbilical cord blood stem cell transplantation through hepatic artery perfusion ,before and after transplantation ,the total number of white blood cells ,percentage of neutrophils and lymphocytes and the change of coagulation function were retrospectively analyzed .Results 8 cases had splenectomy in patients with cirrhosis ,the total number of peripheral blood leukocytes and platelet levels were in normal range;42 patients with liver cirrhosis before um-bilical cord blood stem cell transplantation , the total number of white blood cell , platelet count is below the normal ,after transplantation the number of peripheral blood leukocytes and platelets number , neutrophil per-centage increased in general , lymphocyte percentage declined;Postoperative week 16 total number of white blood cells,neutrophil percentage,lymphocyte percentage was (4.01 ±1.02) ×109/L,(58.7 ±7.2)%, (35.8 ±6.3)%,respectively,compared with the preoperative difference was significantly improved (P <0.05);50 cases of patients with cirrhosis after umbilical cord blood stem cell transplantation ,blood coagula-tion function was improved significantly ( P<0 .05 ) .Conclusion The umbilical cord blood stem cell trans-plantation can improve the blood coagulation function in patients with liver cirrhosis ,improve the total number of white blood cells ,lymphocytes percentage and lower neutrophil percentage .
