目的:探讨糖尿病自身抗体联合检测在糖尿病分型中的应用价值,为正确分型糖尿病提供理论依据。方法选择我院内分泌科收治的126例糖尿病患者,其中1型糖尿病组51例,2型糖尿病组75例,同时选择健康对照组60例,采用免疫印迹试剂法检测胰岛细胞抗体(ICA)、胰岛素自身抗体(IAA)和谷氨酸脱羧酶抗体(GADA)表达情况,计算阳性率,并对结果进行比较分析。结果ICA、IAA 和 GADA 在1型糖尿病患者中的阳性率分别为35.3%、31.4%和60.8%,显著高于2型糖尿病患者的1.3%、1.3%、2.7%及对照组的1.3%、0.0%、0.0%,均有显著性差异(P <0.05)。ICA/IAA/GADA、GADA/ICA、GADA/IAA 三种联合检测组合对1型糖尿病诊断的敏感性分别为92.2%、84.3%和82.4%,均显著高于单一抗体检测,均有显著性差异(P <0.05)。结论检测血清 ICA、IAA 和 GA-DA 能够为正确鉴别1型糖尿病和2型糖尿病提供重要依据,联合检测能够提高诊断阳性率,避免漏诊,值得临床推广和普及。

목적:탐토당뇨병자신항체연합검측재당뇨병분형중적응용개치,위정학분형당뇨병제공이론의거。방법선택아원내분비과수치적126례당뇨병환자,기중1형당뇨병조51례,2형당뇨병조75례,동시선택건강대조조60례,채용면역인적시제법검측이도세포항체(ICA)、이도소자신항체(IAA)화곡안산탈최매항체(GADA)표체정황,계산양성솔,병대결과진행비교분석。결과ICA、IAA 화 GADA 재1형당뇨병환자중적양성솔분별위35.3%、31.4%화60.8%,현저고우2형당뇨병환자적1.3%、1.3%、2.7%급대조조적1.3%、0.0%、0.0%,균유현저성차이(P <0.05)。ICA/IAA/GADA、GADA/ICA、GADA/IAA 삼충연합검측조합대1형당뇨병진단적민감성분별위92.2%、84.3%화82.4%,균현저고우단일항체검측,균유현저성차이(P <0.05)。결론검측혈청 ICA、IAA 화 GA-DA 능구위정학감별1형당뇨병화2형당뇨병제공중요의거,연합검측능구제고진단양성솔,피면루진,치득림상추엄화보급。
Objective To explore the diagnostic value of detection of diabetes antibodies in the classification of dia-betes mellitus and provide a theoretical basis for the correct typing of diabetes mellitus.Methods 126 cases of diabetic patients were hospitalized in the Department of Endocrinology,including 51 cases of type 1 diabetes,75 cases of type 2 diabetes,and 60 healthy controls were detected as control group.Islet cell antibodies (ICA),insulin autoantibody (IAA),glutamic acid decarboxylase antibody (GADA)expression and the positive rate were tested.Results The posi-tive rates of ICA,IAA,GADA in patients with type 1 diabetes were 35.3%,31.4%,60.8%,were significantly higher than 1.3%,1.3%,2.7% and 1.3% in patients with type 2 diabetes (P <0.05).The sensitivity of combined detection of antibodies ICA/IAA/GADA,GADA/ICA and GADA/IAA in the diagnosis of type 1 diabetes were 92.2%,84.3%, <br> 82.4%,were significantly higher than those of single detection (P <0.05).Conclusion The detection of serum ICA, IAA,GADA can provide an important basis for the correct identification of type 1 diabetes and type 2 diabetes.Joint de-tection can improve the positive rate of diagnosis and avoid misdiagnosis,is worth the clinical promotion and populariza-tion.
