目的:探讨采用尺神经松解皮下前置术治疗肱骨远端陈旧性骨折致肘管综合征的临床疗效。方法对103例患者(轻度16例,中度37例,重度50例)采用尺神经松解皮下前置术治疗。观察患者的尺神经卡压症状和体征恢复情况以及肌电图的结果。结果患者均获随访,时间14~20个月。患者体征均有明显改善,根据中华医学会手外科学会上肢部分功能试用评定标准:优55例,良37例,可9例,差2例,优良率为89.3%。结论采用尺神经松解皮下前置术治疗肱骨远端陈旧性骨折致肘管综合征,手术方式简易,临床疗效好。肌电图检查具有一定的随访价值,肘管指数对手术方式的选择具有一定的实用价值。

Objective To investigate the clinical effects of anterior subcutaneous transposition of the ulnar nerve in the treatment of cubital tunnel syndrome caused by old fracture of the distal humerus. Methods 103 cases (16 cases in mild,37 cases in moderate,50 cases in severe) were treated by anterior subcutaneous transposition of the ulnar nerve. Recovery of symptom and sign of the ulnar nerve entrapped and EMG results were recorded. Results All pa-tients were followed up for 14 ~20 months. According to the trial assessment standards of upper limb function in Hand Surgery Association, 55 cases were graded in excellent, 37 in good, 9 in fair and 2 in poor, the excellent and good rates was 89. 3%. Conclusions The treatment of cubital tunnel syndrome caused by old fracture of the distal humerus using anterior subcutaneous transposition of the ulnar nerve has easier surgical approach and better clinical efficacy. EMG has a certain value in follow-up. Elbows index has a certain value in the treatment options.
