目的：探讨临床药师对骨科清洁手术预防用抗菌药物实施药学干预的效果，旨在促进临床合理用药。方法回顾性抽取该院骨科2012年8月-2013年1月手术病历120例作为非干预组，制定一系列干预措施后，选取2013年2月-2013年7月手术病历120例作为干预组，对2组患者预防用抗菌药物的情况进行对比分析。结果干预组头孢唑啉和头孢呋辛的使用率由0．0％分别上升至43．3％和22．5％，抗菌药物使用合理率由干预前的0．0％上升为68．3％（P＜0．01），预防用人均抗菌药物费用也由干预前的824．22元下降为179．79元（ P＜0．01）。结论临床药师对骨科I类切口手术实施药学干预能够提高合理用药水平，增强临床医生合理使用抗菌药物的意识，降低患者医疗费用。

Objective To evaluate the efficacy of pharmaceutical intervention on the prophylactic use of antibiotics in the orthopedic clean operation in an effort to promote clinical rational use of drug .Methods 120 medical records of surgical patients of the orthopedic clean operation in our hospital from Aug .2012 to Jan.2013 by a retrospective extracting method were assigned to non-intervention group,and another 120 medical records of surgical patients in the same hospital from Feb .2013 to Jul.2013 after implementation of a series of pharmaceutical intervention measures were assigned to intervention group .The prophylactic use of antibiotics was compared be-tween the two groups .Results The usage frequency of cefamedin and cefuroxime in the intervention group increased from 0.0%to 43. 3%and 22.5%.The rate of rational use of antibiotics increased from 0.0%to 68.3%(P<0.01);average antibiotic cost decreased from 824.22 yuan to 179.79 yuan(P<0.01).Conclusions The series pharmaceutical intervention of orthopedic I type incision oper -ation by clinical pharmacist improves the level of rational use of drugs ,enhances the consciousness on clinical rational use of antibiot-ics,reduces the patient),s medical costs and playes a positive role to promote safe ,effective,economical,and rational use of antimicro-bial drugs .
