目的：探讨奥布卡因凝胶联合心理干预在特殊人群（宫口无扩张史的剖宫产者，宫颈经微波、LEEP等手术治疗者）放置宫内节育器的临床效应。方法将126例特殊人群分成两组，观察组放置宫内节育器术中加用奥布卡因凝胶，并辅以心理干预，对照组常规行宫内节育器放置术。观察两组宫颈软化、疼痛程度、手术时间、心脑综合征、手术满意度差异。结果观察组总有效率高于对照组，差异有统计学意义（χ2＝50．958，P＜0．01），而手术时间较对照组缩短（t＝10．572，P＜0．001），疼痛评分低于对照组（t＝27．284，P＜0．001），心脑综合征发生率低于对照组（χ2＝8．818，P＜0．05）。观察组手术满意度95．31％，对照组手术满意度67．74％，两组比较差异有统计学意义（χ2＝16．04，P＜0．01）。结论奥布卡因凝胶联合心理干预应用于特殊人群置宫内节育器术中，能有利于手术操作，缩短手术时间，减轻疼痛，提高患者满意度。

목적：탐토오포잡인응효연합심리간예재특수인군（궁구무확장사적부궁산자，궁경경미파、LEEP등수술치료자）방치궁내절육기적림상효응。방법장126례특수인군분성량조，관찰조방치궁내절육기술중가용오포잡인응효，병보이심리간예，대조조상규행궁내절육기방치술。관찰량조궁경연화、동통정도、수술시간、심뇌종합정、수술만의도차이。결과관찰조총유효솔고우대조조，차이유통계학의의（χ2＝50．958，P＜0．01），이수술시간교대조조축단（t＝10．572，P＜0．001），동통평분저우대조조（t＝27．284，P＜0．001），심뇌종합정발생솔저우대조조（χ2＝8．818，P＜0．05）。관찰조수술만의도95．31％，대조조수술만의도67．74％，량조비교차이유통계학의의（χ2＝16．04，P＜0．01）。결론오포잡인응효연합심리간예응용우특수인군치궁내절육기술중，능유리우수술조작，축단수술시간，감경동통，제고환자만의도。
Objective To investigate the clinical effect of oxybuprocaine gel combined with psychological intervention on special population when putting intrauterine device ( IUD) ( women with history of cesarean delivery but without previous record of cervical dilations , women with cervical treatment by microwave or LEEP operations ) .Methods Totally 126 cases of special population were divided into observation group and control group .The observation group placed IUD with oxybuprocaine gel and psychological intervention , while the control group placed IUD with conventional method .The differences in cervical softening , pain degree , operation time , cardio-cerebral syndrome and operation satisfaction were observed .Results Total effective rate of the observation group was significantly higher than that of the control group (χ2 =50.958, P<0.01).The operation time was significantly shorter (t=10.572, P<0.001), the pain score was lower (t=27.284,P<0.001), and the incidence of cardio-cerebral syndrome was significantly lower (χ2 =8.818, P<0.05) in the observation group than the control group .The operation satisfaction degree of the observation group was 95.31%, which was higher than that (67.74%) of the control group, and the difference was significant (χ2 =16.04,P<0.01).Conclusion Oxybuprocaine gel combined with psychological intervention is helpful to shorten operation time , relieve pain and improve patients ’ satisfaction degree for the special population at the course of taking IUD .
