目的：探讨超声引导下粗针活检（ultrasound-guided core-needle biopsy，US-CNB）在诊断甲状腺结节中的应用价值。方法将术前超声诊断为甲状腺恶性结节的120例患者分为两组，观察组60例，采用超声引导下粗针活检；对照组60例，采用超声引导下细针穿刺细胞学检查（fine needle aspiration cytology ，FNAC）。比较两组病例取材满意度、诊断准确性和安全性。结果观察组取材满意度、诊断准确性明显高于对照组，差异具有统计学意义（P＜0.05）。两组敏感性、特异性、阳性预测值、阴性预测值和并发症比较，差异无统计学意义（P＞0.05）。结论超声引导下粗针活检甲状腺结节取材满意度和诊断准确性高于细针活检，同时也安全可靠，更值得临床推广应用。
목적：탐토초성인도하조침활검（ultrasound-guided core-needle biopsy，US-CNB）재진단갑상선결절중적응용개치。방법장술전초성진단위갑상선악성결절적120례환자분위량조，관찰조60례，채용초성인도하조침활검；대조조60례，채용초성인도하세침천자세포학검사（fine needle aspiration cytology ，FNAC）。비교량조병례취재만의도、진단준학성화안전성。결과관찰조취재만의도、진단준학성명현고우대조조，차이구유통계학의의（P＜0.05）。량조민감성、특이성、양성예측치、음성예측치화병발증비교，차이무통계학의의（P＞0.05）。결론초성인도하조침활검갑상선결절취재만의도화진단준학성고우세침활검，동시야안전가고，경치득림상추엄응용。
Objective To discuss the application value of ultrasound-guided core-needle biopsy (US-CNB) in the diagnosis of thy-roid nodule. Methods 120 cases with thyroid malignant nodules diagnosed by ultrasound before operation were divided into 2 groups:observation group and control group, 60 in each;US-CNB was performed on cases in observation group while fine needle aspiration cy-tology (FNAC) on cases in control group;the satisfactory rate of biopsy, the accuracy of diagnosis and the safety in the 2 groups were compared. Results The satisfactory rate and the accuracy of diagnosis in observation group were much higher than those in control group, the difference was of statistical significance (P<0.05);there existed no statistical difference in sensitivity, specificity, positive pre-dictive value, negative predictive value and the occurrence of complications between the 2 groups (P>0.05). Conclusions US-CNB is of better satisfactory rate of biopsy and higher accuracy of diagnosis than FNAC, it is safe and reliable, worthy of wider application in clinic.
