目的：探讨子宫肉瘤的MRI表现及特征，并与病理学对照，以提高对该类疾病的诊断及鉴别诊断能力。方法分析14例经病理证实的子宫肉瘤的MRI影像资料，其中10例行MRI平扫和增强，4例行MRI平扫。结果6例为子宫内膜间质肉瘤，4例为平滑肌肉瘤，4例为癌肉瘤，14例均表现为子宫体积增大，宫腔内巨大软组织肿块影，病灶信号欠均匀，10例增强后分别呈中度或明显不均匀强化。结论 MRI平扫及动态增强扫描并结合临床资料可以提高对子宫肉瘤的正确诊断率，增强扫描可以准确判断肿瘤的解剖部位及对邻近组织的侵犯情况，但最终确诊仍需靠病理学检查。
목적：탐토자궁육류적MRI표현급특정，병여병이학대조，이제고대해류질병적진단급감별진단능력。방법분석14례경병리증실적자궁육류적MRI영상자료，기중10례행MRI평소화증강，4례행MRI평소。결과6례위자궁내막간질육류，4례위평활기육류，4례위암육류，14례균표현위자궁체적증대，궁강내거대연조직종괴영，병조신호흠균균，10례증강후분별정중도혹명현불균균강화。결론 MRI평소급동태증강소묘병결합림상자료가이제고대자궁육류적정학진단솔，증강소묘가이준학판단종류적해부부위급대린근조직적침범정황，단최종학진잉수고병이학검사。
Objective To investigate MR imaging of uterine sarcomas, and with pathological control, in order to improve the capability of diagnosis and differential diagnosis. Methods MR imaging of uterine sarcomas in 14 cases proved by surgery and pathology were retrospectively reviewed. 10 cases were performed with MR routine and enhanced scan, and 4 case was performed with MR routine scan. Results Of the 14 patients, 6 cases were endometrial stromal sarcomas, 4 cases were leiomyosarcomas and 4 cases were malignant mixed mesodermal tumors. All the tumors were appeared as intrauterine huge soft tissue mass with heterogeneous signal intensity. 10 cases showed heterogeneous enhancement moderately or markedly. Conclusion Routine and dynamic contrast-enhanced MRI scans and clinical data can increase the rate of correct diagnosis of uterine sarcoma, enhanced scan is useful in defining accurately the location and extent of a lesion, but the definitive diagnosis can be made ultimately by pathological examination.
