目的对比桡骨远端关节内骨折保守治疗和手术治疗的疗效分析。方法我院60例桡骨远端关节内骨折，其中手术组28例，保守治疗组32例，观察两组骨折愈合时间、愈合质量、手术并发症及腕关节功能。结果两组骨折愈合时间对比无统计学意义，愈后X线评分手术组0.156，保守治疗组0.854，差异有统计学意义。保守治疗8例出现并发症，手术组仅1例。 Dienst腕关节功能评分保守治疗组优2例，良10例，可11例，差9例，手术治疗组优6例，良12例，可10例，差0例。结论桡骨远端关节内骨折，手术治疗和保守治疗对骨折愈合时间影响不大。保守治疗更易导致并发症的发生，且手术治疗能获得更好的腕关节功能，故手术治疗效果明显优于保守治疗。

목적대비뇨골원단관절내골절보수치료화수술치료적료효분석。방법아원60례뇨골원단관절내골절，기중수술조28례，보수치료조32례，관찰량조골절유합시간、유합질량、수술병발증급완관절공능。결과량조골절유합시간대비무통계학의의，유후X선평분수술조0.156，보수치료조0.854，차이유통계학의의。보수치료8례출현병발증，수술조부1례。 Dienst완관절공능평분보수치료조우2례，량10례，가11례，차9례，수술치료조우6례，량12례，가10례，차0례。결론뇨골원단관절내골절，수술치료화보수치료대골절유합시간영향불대。보수치료경역도치병발증적발생，차수술치료능획득경호적완관절공능，고수술치료효과명현우우보수치료。
Objective To compare the ef icacy of intra-articular distal radial fracture by conservative treatment and surgical treatment.Methods 60 patients with intra-articular distal radial fracture treated by dif erent methods ,of which 32 cases of conservative treatment of closed reduction, open reduction and internal fixation surgery of 28 cases. Healing time, reduction quality and functional recovery were observed.Results There is no statistical significance in the healing time between two groups. The reduction quality was measured and scored by X-ray after fracture healing. The conservative group average:0.854, and the surgical group average: 0.156.the dif erence between two group was statistical y significant. Appearance of complication was in 8 cases in conservative group and 1 case in surgical group. According to the Dienst Criterion, 2 cases were rated as excel ent and 10 as good and 11 as faie and 9 as poor in the conservative group. And 6 cases were rated as excel ent and 12 as good and 10 as fair and 0 as poor in surgical group. The dif erence between two group was significant.Conclusion There was lit le ef ect on the fracture healing time between two group for intra-articular distal radial fractures. Conservative treatment was easier with complications than surgical treatment. Wrist function would be bet er in surgical treatment. So intra-articular distal radial fracture treated by surgical treatment had bet er ef ect than conservative treatment.
