目的观察替比夫定治疗慢性乙型肝炎的疗效、影响疗效的因素及不良反应。方法对替比夫定初始治疗的96例慢性乙型肝炎患者进行回顾性研究，评价治疗48w时的应答指标，并对治疗前ALT、HBV-DNA水平、HBeAg状态分别分组进行分析。结果完成48周疗程的91例患者中病毒学应答82例（90%），生化学应答81例（90%），完全应答41例（45%）、部分应答43例（47%）、无应答7例（8%）；61例HBeAg阳性患者中 HBeAg阴转26例（43%），出现 HBeAb12例（20%）；所有HBV-DNA转阴的患者均未出现病毒学反弹。有11例(33%)患者出现不同程度的CK升高,其中2例有肌痛。结论替比夫定治疗慢性乙型肝炎疗效可靠，安全性好,HBV DNA水平低、HBeAg阴性患者联合应答率高。

Objective To investigate the ef icacy of telbivudine treatment of chronic hepatitis B, the factors that af ect the ef icacy and adverse reactions. Methods conducted a retrospective study of 96 cases of telbivudine treatment-na?ve patients with chronic hepatitis B, evaluation of treatment response indicators in the 48 weeks of pre-treatment ALT and HBV-DNA level, HBeAg status were grouped for analysis. Results 91 patients completed 48 weeks of treatment virologic response in 82 patients (90%), biochemical response in 81 patients (90%), 41 cases (45%) complete response, partial response in 43 patients (47%), no response 7 cases (8%);HBeAg negative in 26 cases (43%) of 61 patients with HBeAg-positive patients, appear HBeAb12 patients (20%);Al HBV-DNA negative patient experienced virologic rebound. 11 cases (33%) patients with varying degrees of CK increased, including two cases of myalgia. Conclusion Telbivudine treatment of chronic hepatitis B and reliable security, HBV DNA levels, the combined response rate of HBeAg-negative patients.
