目的:探讨成人部分性癫痫患者的睡眠特征。方法对初诊断的45例成人部分性癫痫患者(患者组)和40名健康者(对照组)行睡眠脑电图监测,分析其睡眠参数。用Epworth嗜睡量表(ESS)行白天嗜睡状况评价。比较两组睡眠参数和ESS评分的差异,并分析它们之间的关系。结果患者组ESS评分高于对照组(P<0.05);与对照组相比,患者组总睡眠时间减少(P<0.05),睡眠效率下降(P<0.05),非快速眼动睡眠(NREM)1、2期及清醒期占总睡眠时间百分比增加(P<0.05),NREM慢波睡眠期(3+4期)占总睡眠时间百分比下降(P<0.05);两组总记录时间、睡眠潜伏期、快速眼动睡眠(REM)潜伏期和觉醒次数无显著性差异(P>0.05)。患者组中,每月发作≥3次者ESS评分明显高于每月发作<3次者(P<0.01);与每月发作<3次者比较,每月发作≥3次者总睡眠时间减少(P<0.05),睡眠效率下降(P<0.05),NREM 1、2期及清醒期占总睡眠时间的百分比增加(P<0.05),NREM 3+4期占总睡眠时间百分比下降(P<0.05),REM期占总睡眠时间百分比下降(P<0.05);两组间总记录时间、睡眠潜伏期、REM潜伏期和觉醒次数均无显著性差异(P>0.05)。患者组的ESS评分与睡眠效率(r=-0.49, P<0.05)、NREM 3+4期占总睡眠时间百分比(r=-0.54, P<0.05)呈负相关,与记录的其余睡眠参数不相关(P>0.05)。结论成年部分性癫痫患者有睡眠结构异常,白天嗜睡程度增高;癫痫发作可加重睡眠结构异常和白天嗜睡程度;睡眠参数异常与ESS评分有一定的相关性。

목적:탐토성인부분성전간환자적수면특정。방법대초진단적45례성인부분성전간환자(환자조)화40명건강자(대조조)행수면뇌전도감측,분석기수면삼수。용Epworth기수량표(ESS)행백천기수상황평개。비교량조수면삼수화ESS평분적차이,병분석타문지간적관계。결과환자조ESS평분고우대조조(P<0.05);여대조조상비,환자조총수면시간감소(P<0.05),수면효솔하강(P<0.05),비쾌속안동수면(NREM)1、2기급청성기점총수면시간백분비증가(P<0.05),NREM만파수면기(3+4기)점총수면시간백분비하강(P<0.05);량조총기록시간、수면잠복기、쾌속안동수면(REM)잠복기화각성차수무현저성차이(P>0.05)。환자조중,매월발작≥3차자ESS평분명현고우매월발작<3차자(P<0.01);여매월발작<3차자비교,매월발작≥3차자총수면시간감소(P<0.05),수면효솔하강(P<0.05),NREM 1、2기급청성기점총수면시간적백분비증가(P<0.05),NREM 3+4기점총수면시간백분비하강(P<0.05),REM기점총수면시간백분비하강(P<0.05);량조간총기록시간、수면잠복기、REM잠복기화각성차수균무현저성차이(P>0.05)。환자조적ESS평분여수면효솔(r=-0.49, P<0.05)、NREM 3+4기점총수면시간백분비(r=-0.54, P<0.05)정부상관,여기록적기여수면삼수불상관(P>0.05)。결론성년부분성전간환자유수면결구이상,백천기수정도증고;전간발작가가중수면결구이상화백천기수정도;수면삼수이상여ESS평분유일정적상관성。
Objective To explore the sleep characteristics of adult partial epilepsy patients. Methods 45 patients newly diagnosed as adult partial epilepsy and 40 cases of healthy controls were monitored with sleep encephalogram (EEG), the sleep parameters were ana-lyzed. Epworth Sleepiness Scale (ESS) was used to evaluate the daytime sleepiness next day. The sleep parameters and ESS score were com-pared between 2 groups, and the correlation between sleep parameters and ESS score in the patients were analyzed. Results The ESS score was higher in the patients than in the controls (P<0.05);Compared with the controls, the total sleep time decreased (P<0.05), the sleep effi-ciency reduced (P<0.05), the percentage of non-rapid eye movement sleep (NREM) stages 1 and 2 and the wakefulness stage increased (P<0.05), the percentage of NREM stages 3 and 4 decreased (P<0.05) in the patients; There was no significant difference in total recording time, sleep latency, REM latency, and arousal times between the patients and the controls (P>0.05). The ESS score was higher in the patients who attacked 3 times a month or more (group A) than less than 3 (group B). Compared with group B, the total sleep time decreased (P<0.05), the sleep efficiency reduced (P<0.05), the percentage of NREM stages 1 and 2 and the wakefulness stage increased (P<0.05), the per-centage of NREM stages 3 and 4, and REM decreased (P<0.05) in group A. There was no significant difference in sleep latency, REM laten-cy and arousal times between 2 groups (P>0.05). The ESS score of the patients was negatively correlated with sleep efficiency (r=-0.49, P<0.05) and the percentage of NREM stages 3 and 4 (r=-0.54, P<0.05), and was not correlated with other sleep parameters (P>0.05). Conclu-sion The sleep architecture of patients with adult partial epilepsy is abnormal, and their daytime sleepiness is more than healthy persons. Epi-leptic seizure can aggravate sleep architecture abnormality and daytime sleepiness. Abnormal sleep parameters are related with ESS score.
