目的:探讨高血糖高渗状态对老年糖尿病患者甲状腺功能及炎性指标的影响。方法选择2012年1月-2014年1月在我院接受治疗的老年糖尿病高血糖高渗状态患者43例作为观察组,另选同期我院体检的老年糖尿病患者30例为对照组。观察两组血糖、糖化血红蛋白( HbA1c)和血浆渗透压,甲状腺功能及血浆炎性指标。结果观察组空腹血糖、HbA1c和血浆渗透压均显著高于对照组(P <0.01)。观察组血浆三碘甲状腺原氨酸(T3)水平显著低于对照组,血浆 C反应蛋白(CRP)和肿瘤坏死因子α(TNF-α)水平显著高于对照组(P <0.01)。观察组血浆 T3水平与 CRP和 TNF-α水平呈现显著负相关(r=-0.743,r=-0.583,P<0.01)。结论高血糖高渗状态可抑制老年糖尿病患者的甲状腺功能,可能是同时存在的全身性炎性反应所致。

목적:탐토고혈당고삼상태대노년당뇨병환자갑상선공능급염성지표적영향。방법선택2012년1월-2014년1월재아원접수치료적노년당뇨병고혈당고삼상태환자43례작위관찰조,령선동기아원체검적노년당뇨병환자30례위대조조。관찰량조혈당、당화혈홍단백( HbA1c)화혈장삼투압,갑상선공능급혈장염성지표。결과관찰조공복혈당、HbA1c화혈장삼투압균현저고우대조조(P <0.01)。관찰조혈장삼전갑상선원안산(T3)수평현저저우대조조,혈장 C반응단백(CRP)화종류배사인자α(TNF-α)수평현저고우대조조(P <0.01)。관찰조혈장 T3수평여 CRP화 TNF-α수평정현현저부상관(r=-0.743,r=-0.583,P<0.01)。결론고혈당고삼상태가억제노년당뇨병환자적갑상선공능,가능시동시존재적전신성염성반응소치。
Objective To explore the effect of hyperglycemic hyperosmolar status on thyroid function and inflam-mation index in elderly patients with diabetes. Methods A total of 43 elderly diabetic patients with hyperglycemic hy-perosmolar status undergoing treatment during January 2012 and January 2014 was chosen as observation group,and 30 elderly diabetic patients taking medical examinations at the same time were chosen as control group. The values of fasting plasma glucose(FPG),glycosylated hemoglobin(HbA1c),plasmic osmotic pressure,thyroid function and inflammation index in the two groups were observed. Results The values of FPG,HbA1c and plasmic osmotic pressure in observation group were significantly higher than those in control group(P<0. 01). In observation group,the triiodothyronine(T3) level was significantly lower,while the levels of C-reactive protein( CRP)and tumor necrosis factor-α( TNF-α)were significantly higher than those in control group(P<0. 01). The T3 level had significantly negative correlation with the levels of CRP and tumor necrosis factorα(TNF-α)in observation group(r= -0. 743,r= -0. 583,P<0. 01). Con-clusion Elderly diabetic patients with hyperglycemic hyperosmolar status may inhibit the level of thyroid function,and systemic inflammatory reaction may be found at the same time.
