目的:对比分析数字乳腺X线摄影、磁共振成像(MRI)及两者联合对乳腺肿瘤良、恶性的诊断。方法136例临床乳腺肿瘤病例,分为X线组(n=50)、MRI组(n=46)、联合组(n=40),分别行乳腺X线摄影、MRI及两者联合检查。以穿刺活检或手术后病理结果检验影像学诊断,总结3种检查方法影像学特点并对比分析3者对乳腺肿瘤良恶性鉴别的优缺点。结果136例患者中,恶性肿瘤占34.6%(47/136),良性肿瘤占65.4%(89/136)。X线组良、恶性肿瘤诊断敏感性、特异性、准确性分别为77.8%、81.3%、80.0%;MRI组分别为83.3%、82.1%、82.0%;联合组分别为85.7%、84.6%、85.0%。联合组在敏感性、特异性及准确性上均高于其他两组(均P<0.05)。结论乳腺X线摄影联合MRI技术提高了乳腺良、恶性肿瘤诊断率及鉴别率。

Objective To compare digital mammography, magnetic resonance imaging (MRI) and combination of two methods in diagnosis of breast tumors. Methods One hundred and thirty six patients with breast tumors underwent imaging ex-aminations, including 50 cases with digital mammography, 46 cases with MRI and 40 cases with both examinations. Pathologic diagnosis was made with the tissues samples obtained by core needle biopsy or surgical resection. Using pathological results as gold standard the accuracy of three methods in differential diagnosis was evaluated. Results Among 136 cases, 47 (34.6%) were pathological y diagnosed as breast cancer and remaining 89 (165.4%) as benign lesions. The sensitivity, specificity and ac-curacy of combination group (85.7%, 84.6%, 85.0%) were significantly higher than those of digital mammography (77.8%, 81.3%, 80.0%) and MRI (83.3%, 82.1%, 82. 0%). Conclusion Digital mammography combined with MRI may increase the differential di-agnostic rate of benign and malignant breast tumors.
