目的:探讨双源CT双能量颅脑血管造影(DE-CTA)在颅内动脉瘤诊断中的临床价值。方法回顾性分析2011年6月至2013年12月我院收治的84例有完整临床资料的脑动脉瘤患者。所有患者术前均行DE-CTA检查诊断脑动脉瘤,其中,19例DE-CTA诊断后经开颅手术夹闭治疗,65例经DSA介入栓塞治疗。所有患者均有治疗后复查DE-CTA资料。 DE-CTA处理完成后,由2位资深放射医师评价图像质量及进行诊断,统计并比较DE-CTA检查与手术及数字减影血管造影(DSA)检查结果。结果84例动脉瘤患者均行DE-CTA检查,共检出动脉瘤99个。19例患者行DE-CTA检查诊断后经开颅手术夹闭治疗证实,均为单发较大动脉瘤破裂出血,手术所见与DE-CTA检查结果相符;65例患者经DSA介入栓塞治疗。 DE-CTA检查结果与手术及DSA所见相近。结论 DE-CTA作为一种快捷、简便、无创而又能准确诊断脑动脉瘤的技术,在临床诊断脑动脉瘤中具有较高的应用价值。

목적:탐토쌍원CT쌍능량로뇌혈관조영(DE-CTA)재로내동맥류진단중적림상개치。방법회고성분석2011년6월지2013년12월아원수치적84례유완정림상자료적뇌동맥류환자。소유환자술전균행DE-CTA검사진단뇌동맥류,기중,19례DE-CTA진단후경개로수술협폐치료,65례경DSA개입전새치료。소유환자균유치료후복사DE-CTA자료。 DE-CTA처리완성후,유2위자심방사의사평개도상질량급진행진단,통계병비교DE-CTA검사여수술급수자감영혈관조영(DSA)검사결과。결과84례동맥류환자균행DE-CTA검사,공검출동맥류99개。19례환자행DE-CTA검사진단후경개로수술협폐치료증실,균위단발교대동맥류파렬출혈,수술소견여DE-CTA검사결과상부;65례환자경DSA개입전새치료。 DE-CTA검사결과여수술급DSA소견상근。결론 DE-CTA작위일충쾌첩、간편、무창이우능준학진단뇌동맥류적기술,재림상진단뇌동맥류중구유교고적응용개치。
Objective To investigate the application of dual energy CTA in the diagnosis of intracranial a-neurysms by using dual-source CT. Methods The imaging data of 84 patients who were confirmed as intracranial a-neurysms by the endovascular treatment or surgery in our hospital from June 2011 to Decemember 2013 were ana-lyzed. Images of dual energy CTA were double blindly reviewed by two radiologists independently. Results Among the 84 patients, 99 intracranial aneurysms were verified by dual energy CTA, 19 cases with intracranial aneurysms were verified by surgery or the endovascular treatment. They were all single aneurysm ,which were completely the same as the diagnosis of dual energy CTA, 65 patients had endovascular treatment ,and 81 aneurysms were verified , which was very closed to the diagnosis of dual energy CTA. Conclusion As a fast, noninvasive method, dual energy CTA can help to diagnose intracranial aneurysms accurately, which has highly application value in the diagnosis of intracranial aneurysms to the clinic.
