目的:研究高血压患者颈动脉硬化斑块形成与高血压脑卒中的相关关系。方法采用高频彩色多普勒超声检测85例高血压并颈动脉硬化斑块形成患者及40名健康人颈动脉的内膜-中膜厚度及斑块形成情况,分析单纯高血压患者与高血压脑卒中患者颈动脉内膜增厚及斑块形成情况。结果85例高血压患者动脉硬化斑块形成共166处,其中硬斑块64处,软斑块102处;管腔狭窄共计30例,其中轻度管腔狭窄11例,中度狭窄12例,重度狭窄7例。以动脉粥样斑块3处以上作为预测患者发生高血压脑卒中,其敏感性80%,特异性82%,阳性预测值83%。结论高频超声能有效检测高血压脑卒中患者的颈动脉斑块形成,为临床预测高血压脑卒中提供有价值的信息。

목적:연구고혈압환자경동맥경화반괴형성여고혈압뇌졸중적상관관계。방법채용고빈채색다보륵초성검측85례고혈압병경동맥경화반괴형성환자급40명건강인경동맥적내막-중막후도급반괴형성정황,분석단순고혈압환자여고혈압뇌졸중환자경동맥내막증후급반괴형성정황。결과85례고혈압환자동맥경화반괴형성공166처,기중경반괴64처,연반괴102처;관강협착공계30례,기중경도관강협착11례,중도협착12례,중도협착7례。이동맥죽양반괴3처이상작위예측환자발생고혈압뇌졸중,기민감성80%,특이성82%,양성예측치83%。결론고빈초성능유효검측고혈압뇌졸중환자적경동맥반괴형성,위림상예측고혈압뇌졸중제공유개치적신식。
Objective To study the correlation of the carotid arteries plaque in patients with hypertension com-plicating acute cerebral accident. Methods To detect the carotid arteries plaque in 85 patients with hypertension and carotid arteries plaque and 40 normal subjects by using color Doppler high-frequency ultrasound. Results There were 166 carotid arteries plaque in 85 cases, included 64 hard plaque and 102 soft plaque , 30 cases had carotid ar-teries stenosis, including 11 cases of slight stenosis, 12 cases of midrange stenosis and 7 cases of severe stenosis. Over 3 carotid arteries plaque could be used to predict acute cerebral accident with higher sensitivity as 80%, speci-ficity as 82%, and positive predictive value as 83%. Conclusion Color Doppler high-frequency ultrasound can be used to measure the carotid arteries plaque in patients with hypertension, which can offer valuable information for pre-dicting the hypertension complicating acute cerebral accident.
