目的:分析探讨原发性高血压血压昼夜节律异常与肾损害的关系。方法选择2012年6月~2014年2月我院收治并行24 h动态血压监测的原发性高血压患者154例,根据昼夜血压监测结果将其分为杓型组51例、非杓型组64例、反杓型组32例和深杓型组7例。观察比较各组患者血压昼夜节律异常情况,并对所有研究对象的尿微量白蛋白(mALB)、血尿素氮(BUN)、血肌酐(SCr)和血尿酸(UA)进行检测。结果4组患者日间血压比较无明显差异( P>0.05),非杓型、反杓型和深杓型组患者夜间舒张压、收缩压均明显高于杓型组,反杓型组患者的24 h脉压差也明显高于杓型组( P<0.05)。4组患者在尿微量白蛋白、血尿素氮和血肌酐上有显著差异( P<0.05),而在血尿酸上的差异则无统计学意义( P>0.05)。结论原发性高血压血压昼夜节律异常与肾损害有关,非杓型、反杓型和深杓型高血压患者有发生早期肾损害的倾向。

목적:분석탐토원발성고혈압혈압주야절률이상여신손해적관계。방법선택2012년6월~2014년2월아원수치병행24 h동태혈압감측적원발성고혈압환자154례,근거주야혈압감측결과장기분위표형조51례、비표형조64례、반표형조32례화심표형조7례。관찰비교각조환자혈압주야절률이상정황,병대소유연구대상적뇨미량백단백(mALB)、혈뇨소담(BUN)、혈기항(SCr)화혈뇨산(UA)진행검측。결과4조환자일간혈압비교무명현차이( P>0.05),비표형、반표형화심표형조환자야간서장압、수축압균명현고우표형조,반표형조환자적24 h맥압차야명현고우표형조( P<0.05)。4조환자재뇨미량백단백、혈뇨소담화혈기항상유현저차이( P<0.05),이재혈뇨산상적차이칙무통계학의의( P>0.05)。결론원발성고혈압혈압주야절률이상여신손해유관,비표형、반표형화심표형고혈압환자유발생조기신손해적경향。
Objective To investigate the relationship between the abnormal circadian rhythm of blood pressure and the renal damage in patients with primary hypertension .Methods A total of 154 patients with primary hypertension who were treated and underwent 24-h ambulatory blood pressure monitoring in our hospital from June 2012 to February 2014 were enrolled in this study .They were divided into four groups with 51 in dipper group , 64 in non-dipper group , 32 in anti-dipper group and 7 in deep-dipper group.The abnormal circadian rhythm of blood pressure was observed and compared among the groups.The urinary micro albumin (mALB), blood urea nitrogen (BUN), se-rum creatinine (SCr) and uric acid (UA) were detected in all patients.Results There was no sig-nificant difference in the daytime blood pressure among the four groups ( P>0.05) .The nocturnal systolic and diastolic blood pressure was significantly higher in non-dipper group , anti-dipper group and deep-dipper group than in dipper group ( P<0.05) .The 24-h pulse pressure was also signifi-cantly increased in anti-dipper group when compared with deep-dipper group ( P<0.05) .Signifi-cant difference was noted in the mALB , BUN and SCr except UA among the four groups (P<0.05) .Conclusion The abnormal circadian rhythm of blood pressure is correlated with the renal damage in patients with primary hypertension .Patients with a non-dipper, anti-dipper or deep-dipper pres-sure profile are more likely to develop early renal damage .
