目的:探讨左卡尼汀( LC)治疗非酒精性脂肪肝( NAFLD)的疗效。方法:62例NAFLD患者,随机分为LC治疗组( LC组)32例和安慰剂对照组30例,LC组予LC口服,每次1 g,早晚饭后各1次,疗程共24周。记录治疗前后患者血清丙氨酸氨基转移酶( ALT)、天冬氨酸氨基转移酶( AST)、γ-谷氨酰转肽酶( GGT)、三酰甘油( TG)、总胆固醇( TC)、C反应蛋白( CRP)、空腹血糖( FBG)、胰岛素( FINS)、胰岛素抵抗指数( HOMA-IR)水平,以及肝脏超声表现。结果:与治疗前和对照组比较,LC组治疗后ALT、AST、GGT、TG,TC、FINS、HOMA-IR值均明显降低(P<0.01),肝脏B超明显改善,总有效率明显高于对照组(P<0.01)。结论:左卡尼汀可有效改善NAFLD患者肝功能、血脂,减轻胰岛素抵抗,能安全有效地用于治疗NAFLD。

목적:탐토좌잡니정( LC)치료비주정성지방간( NAFLD)적료효。방법:62례NAFLD환자,수궤분위LC치료조( LC조)32례화안위제대조조30례,LC조여LC구복,매차1 g,조만반후각1차,료정공24주。기록치료전후환자혈청병안산안기전이매( ALT)、천동안산안기전이매( AST)、γ-곡안선전태매( GGT)、삼선감유( TG)、총담고순( TC)、C반응단백( CRP)、공복혈당( FBG)、이도소( FINS)、이도소저항지수( HOMA-IR)수평,이급간장초성표현。결과:여치료전화대조조비교,LC조치료후ALT、AST、GGT、TG,TC、FINS、HOMA-IR치균명현강저(P<0.01),간장B초명현개선,총유효솔명현고우대조조(P<0.01)。결론:좌잡니정가유효개선NAFLD환자간공능、혈지,감경이도소저항,능안전유효지용우치료NAFLD。
Objective:To explore of the effects of L-carnitine(LC) in the treatment of nonalcoholic fatty liver disease(NAFLD). Methods:Sixty-two patients with NAFLD were randomly divided into the LC treatment group ( LC group ) ( 32 cases ) and placebo control group(30 cases). The LC group were treated with 1 g of LC by oral,twice a day for 24 weeks after breakfast and dinner. The levels of serum alanine aminotransferase ( ALT ) , aspartate aminotransferase ( AST ) ,γ-glutamyltransferase ( GGT ) , triglyceride ( TG ) , total cholesterol(TC),C reactive protein(CRP),fasting plasma glucose(FBG),insulin(FINS),insulin resistance index(HOMA-IR) and liver ultrasound in all patients were recorded before and after treatment. Results:Compared with the before treatment and control group,the levels of ALT,AST and GGT,TG and the TC,FINS and HOMA-IR of LC group decreased significantly(P<0. 01),and the liver B ultrasound was improved obviously after treatment. The total effective rate of the LC group was significantly higher than that in control group(P<0. 01). Conclusions:L-carnitine can effectively improve the liver function and blood lipid,and reduce the insulin resistance of patients with NAFLD,which is a safe and effective drug in the treatment of NAFLD.
