目的 观察孔源性视网膜脱离复位前后相关的自发荧光(autofluorescence,AF)表现,提升对孔源性视网膜脱离复位前后相关特征的理解.方法 对2013年1~8月就诊于山西省眼科医院门诊的局限性原发性孔源性视网膜脱离患者47例48只眼(伴黄斑脱离43只眼,黄斑在位5只眼)成功行巩膜扣带术,所有患眼术前和术后1个月分别行光相干断层扫描(OCT)、脂褐质相关眼底自发荧光检查(FAF)和黑色素相关眼底自发荧光检查(NIA).对比分析FAF和NIA的检查结果.结果 术前12只眼(25%)视网膜脱离边界FAF和NIA均为强荧光,余36只眼(75%) FAF和NIA均未见明显异常荧光.术前所有患眼视网膜脱离的区域FAF和NIA均表现为弱荧光(100%).术后14只眼(29.17%)视网膜复位区域FAF和NIA表现为强荧光;10只眼(20.83%) FAF和NIA表现正常;余24只眼(50%) NIA表现为弱荧光,而FAF则表现不一,其中17只眼(35.42%)为弱荧光,6只眼(12.50%)为强荧光,1只眼(2.08%)正常.术前伴黄斑脱离的43只眼中,术后黄斑FAF均表现为弱荧光(100%),但NIA则表现不一,9只眼(20.93%)不存在黄斑下液,NIA为强荧光;34只眼(79.07%)存在黄斑下液,其中30只眼(88.24%) NIA为强荧光中夹杂点状弱荧光,4只眼(11.76%)NIA为强荧光.结论 FAF与NIA结合可以发现孔源性视网膜脱离边界,评估孔源性视网膜脱离复位后早期黄斑及复位区的相关特征.

Objective To observe the autofluorescence (AF) manifestation and enhance our understanding about characteristics before and after rhegmatogenous retinal detachment (RRD) reattached.Methods The examinations of Optical Coherence Tomography (OCT),lipofuscin-related fundus autofluorescence (FAF) and melanin-related near-infrared fundus autofluorescence (NIA) were performed before and after retina reattached one month in 47 patients (48 eyes) with local primary RRD who underwent successful scleral buckling in Shanxi Eye Hospital from January to August in 2013.Forty-three eyes had RRDs that involved the macula,whereas five eyes had macula-sparing RRDs.Fluorescence based on FAF and NIA examination was comparatively analyzed.Results Both FAF and NIA showed hyperfluorescencent leading edge in twelve eyes (25%) preoperatively,other thirty-six eyes (75%) didn't showabnormal fluorescence.All forty-eight eyes (100%) demonstrated hypofluorescence over the area of retinal detachment in both FAF and NIA.Postoperatively,fourteen eyes (29.17%) showed hyperfluorescence in FAF and NIA in reattached region,ten eyes (20.83%) showed normal.Among twenty-four eyes (50%) with hypofluorescence in NIA in reattached region,seventeen eyes (35.42%) showed hypofluorescence,six eyes (12.50%) showed hyperfluorescence and one eye (2.08%) was normal in FAF.Among forty-three eyes which had RRDs that involved the macula preoperatively,revealed hypofluorescence in FAF (100%) and had different NIA manifestation postoperatively,nine eyes (20.93%) didn't have submacular fluid and showed hyperfluorescence in NIA in macular postoperately,thirty-four eyes (79.07%) had submacular fluid.Among the thirty-four eyes,thirty eyes (88.24%) showed hypofluorescencent dots involved in hyperfluorescence and four eyes (11.76%) showed hyerfluorescence in NIA in macular postoperately.Conclusions FAF combined with NIA can reveal demarcation of the extent of RRD and evaluate early characteristics of macular and reattached region after RRD reattached.
