目的：探讨伊立替康（CPT-11）和拓扑替康（TPT）治疗老年小细胞肺癌的有效性与安全性。方法回顾性分析39例老年小细胞肺癌，其中CPT-11组22人，TPT组17人，CPT-11组第1 d静脉注射CPT-11300 mg· m-2，TPT组第1～5 d静脉注射TPT 1.25～1.50 mg· m-2，均以3周为1个周期，2组患者分别予化疗2～6个周期；比较2组用药的安全性。结果2组间在近期疗效和远期疗效上差异均无统计学意义（ P＞0.05）；TPT组白细胞减少例数高于CPT-11组，差异有统计学意义（ P＜0.05）；而在延迟性腹泻中，CPT-11组例数高于TPT组，差异有统计学意义（ P＜0.05）；在贫血、血小板减少、恶心呕吐及肝功能异常中，2组差异无统计学意义（ P＞0.05）。结论 CPT-11组及TPT组化疗的近期、远期疗效相似；血液毒性反应中， CPT-11优于TPT组；肠道反应中， TPT组优于CPT-11组。
목적：탐토이립체강（CPT-11）화탁복체강（TPT）치료노년소세포폐암적유효성여안전성。방법회고성분석39례노년소세포폐암，기중CPT-11조22인，TPT조17인，CPT-11조제1 d정맥주사CPT-11300 mg· m-2，TPT조제1～5 d정맥주사TPT 1.25～1.50 mg· m-2，균이3주위1개주기，2조환자분별여화료2～6개주기；비교2조용약적안전성。결과2조간재근기료효화원기료효상차이균무통계학의의（ P＞0.05）；TPT조백세포감소례수고우CPT-11조，차이유통계학의의（ P＜0.05）；이재연지성복사중，CPT-11조례수고우TPT조，차이유통계학의의（ P＜0.05）；재빈혈、혈소판감소、악심구토급간공능이상중，2조차이무통계학의의（ P＞0.05）。결론 CPT-11조급TPT조화료적근기、원기료효상사；혈액독성반응중， CPT-11우우TPT조；장도반응중， TPT조우우CPT-11조。
Objective To compare the efficacy and safety between irino-tecan (CPT -11) and topotecan (TPT) in the treatment of small cell lung cancer in elderly patients.Methods Data of 39 cases of elderly pathologically confirmed small cell lung cancer ( SCLC ) hospitalized in Xinjiang uygur autonomous region people′s hospital were retrospectively analyzed , in which 22 cases received CPT-11 while the other 17 cases received TPT as chemotherapy .The dose of CPT-11 was 300 mg· m-2 in the first day , and TPT 1.25-1.50 mg· m-2 from the first to the fifth day, 3 weeks was a treatment course .Every patient in two groups accept-ed 2 to 6 periods of chemotherapy ,and the toxicities and curative efficacy were compared between the two groups.Results There was no statisti-cal difference in the curative efficacy or long -term efficacy between the two groups ( P>0.05 ).A higher occurrence rate of leukopenia was ob-served in TPT group compared with the CPT -11 group ( P <0.05 ) , while the ratio of delayed diarrhea showed a reverse result.There were no significant differences in the incidence rates of anemia , thrombocytope-nia, nausea , vomiting and abnormal liver function ( P>0.05 ).Conclu-sion CPT-11 and TPT have similar curative efficacy or long-term ef-ficacy on elderly SCLC patients .The former has more severe gastrointes-tinal reaction , but the latter has more severe hematotoxicity.
