目的：探讨防旋股骨近端髓内钉（PFNA）治疗高龄股骨转子间骨折的临床疗效。方法：将2008年1月-2012年3月本院收治的高龄股骨转子间骨折122例，按照不同固定方式将其分为动髋螺钉（DHS）治疗57例和股骨近端髓内钉（PFNA）治疗65例，比较两组术中出血量、平均手术时间、术后骨折临床愈合时间、髋关节的功能。结果：PFNA组骨折术中出血量、手术时间明显少于DHS组，两组比较差异有统计学意义（P<0.05）；所有病例均获得随访，平均12个月，在骨折临床愈合时间及髋关节的功能方面也优于DHS组，两组比较差异有统计学意义（P<0.05）。结论：PFNA内固定术治疗高龄股骨转子间骨折临床疗效满意。
목적：탐토방선고골근단수내정（PFNA）치료고령고골전자간골절적림상료효。방법：장2008년1월-2012년3월본원수치적고령고골전자간골절122례，안조불동고정방식장기분위동관라정（DHS）치료57례화고골근단수내정（PFNA）치료65례，비교량조술중출혈량、평균수술시간、술후골절림상유합시간、관관절적공능。결과：PFNA조골절술중출혈량、수술시간명현소우DHS조，량조비교차이유통계학의의（P<0.05）；소유병례균획득수방，평균12개월，재골절림상유합시간급관관절적공능방면야우우DHS조，량조비교차이유통계학의의（P<0.05）。결론：PFNA내고정술치료고령고골전자간골절림상료효만의。
Objective:To investigate the clinical efficacy of proximal femoral nail anti-rotation(PFNA)in treatment of intertrochanteric fractures in elderly patients.Method:One hundred and twenty-two cases of elderly femoral fracture from January 2008 to March 2012 in the hospital were randomly divided into the dynamic hip screw(DHS)for 57 cases and the proximal femoral nail(PFNA)for 65 cases. The bleeding in operation,average operation time,postoperative fracture healing time,the function of the hip joint of the two groups were compared. Result:The fracture bleeding volume,operation time in PFNA group were obviously better than that of DHS group,the difference were statistically significant(P<0.05);all cases were followed up, average 12 months,the fracture healing time and hip function were better than that of DHS group,the difference were statistically significant(P<0.05). Conclusion:The clinical effect of PFNA internal fixation in the treatment of femoral intertrochanteric fractures is satisfied.
