目的：评价射频消融（ RFA）治疗局部晚期胰头癌的临床疗效。方法：将87例局部晚期胰头癌患者分为两组，一组给予RFA联合胆肠及胃肠旁路吻合术（RFA组，n＝42），另一组给予单纯胆肠及胃肠吻合术（对照组， n＝45）；随访3～50个月，比较治疗前与治疗3个月后血清CA19-9和CA242水平的变化，应用Kaplan-Meier法绘制生存曲线并进行log-rank检验。结果：RFA组治疗3个月后血清CA19-9和CA242水平均较对照组下降更明显（ t＝10．589、6．130，P＜0．001）。两组患者生存曲线差异有统计学意义（χ2＝33．152，P＜0．001）；RFA组1、2、3 a生存率分别为77．9％、28．7％、2．4％，中位生存期为21个月（95％置信区间17．86～24．14个月）；对照组1、2、3 a生存率分别为22．9％、2．2％、0．0％，中位生存期为10个月（95％置信区间8．90～11．10个月）。结论：RFA能够有效延长局部晚期胰头癌患者的生存期。
목적：평개사빈소융（ RFA）치료국부만기이두암적림상료효。방법：장87례국부만기이두암환자분위량조，일조급여RFA연합담장급위장방로문합술（RFA조，n＝42），령일조급여단순담장급위장문합술（대조조， n＝45）；수방3～50개월，비교치료전여치료3개월후혈청CA19-9화CA242수평적변화，응용Kaplan-Meier법회제생존곡선병진행log-rank검험。결과：RFA조치료3개월후혈청CA19-9화CA242수평균교대조조하강경명현（ t＝10．589、6．130，P＜0．001）。량조환자생존곡선차이유통계학의의（χ2＝33．152，P＜0．001）；RFA조1、2、3 a생존솔분별위77．9％、28．7％、2．4％，중위생존기위21개월（95％치신구간17．86～24．14개월）；대조조1、2、3 a생존솔분별위22．9％、2．2％、0．0％，중위생존기위10개월（95％치신구간8．90～11．10개월）。결론：RFA능구유효연장국부만기이두암환자적생존기。
Aim:To evaluate the clinical efficacy of radiofreqency ablation ( RFA) on locally advanced pancreatic head cancer(LAPHC).Methods:A total of 87 LAPHC patients were divided into RFA group (n=42) and control group(n=45),and followed up for 3-50 months.The serum CA19-9 and CA242 levels preoperation and 3 months after operation were determined .Survival curve was drown by the Kaplan-Meier method .Results:The descent range of serum CA 19-9 and CA242 levels of RFA group 3 months after operation were more than that of the control group ( t=10.589,6.130,P<0.001).The survival curve of RFA group differed from that of the control group (χ2 =33.152,P<0.001).The 1-,2-and 3-year overall cumulative survival rates in RFA group and control group were 77.9%,28.7%,2.4%and 22.9%,2.2%, 0.0%,respectively;the median survival time was 21 months(95%CI was 17.86-24.14 months) in RFA group and 10 months(95%CI was 8.90-11.10 months) in control group,respectively.Conclusion: RFA can effectively prolong sur-vival time of patients with LAPHC .
