目的:探讨补充维生素 D 对慢性阻塞性肺疾病(COPD)患者骨密度及炎症因子的影响。方法:将维生素 D 缺乏的 COPD 患者随机分成观察组175例和对照组175例,两组常规治疗相同,观察组加用骨化三醇胶丸(0.25μg·d-1)治疗,比较两组治疗3个月后骨密度、炎症因子及肺功能的差别。结果:①治疗后,观察组血钙、25(OH)D、骨密度和 T 值均显著高于对照组,血磷、TNF-α和 IL-6均显著低于对照组,差异均有统计学意义(P <0.05)。②治疗后,观察组第 l 秒用力呼气容积(FEV1)、第 l 秒用力呼气容积/肺功能包括用力肺活量(FEV1/FVC)、单位肺泡一氧化碳弥散量(DLCO/VA)均显著高于对照组,差异均有统计学意义(P <0.05)。结论:补充维生素D 可提高 COPD 患者骨密度,降低炎症因子水平,并改善肺功能。

목적:탐토보충유생소 D 대만성조새성폐질병(COPD)환자골밀도급염증인자적영향。방법:장유생소 D 결핍적 COPD 환자수궤분성관찰조175례화대조조175례,량조상규치료상동,관찰조가용골화삼순효환(0.25μg·d-1)치료,비교량조치료3개월후골밀도、염증인자급폐공능적차별。결과:①치료후,관찰조혈개、25(OH)D、골밀도화 T 치균현저고우대조조,혈린、TNF-α화 IL-6균현저저우대조조,차이균유통계학의의(P <0.05)。②치료후,관찰조제 l 초용력호기용적(FEV1)、제 l 초용력호기용적/폐공능포괄용력폐활량(FEV1/FVC)、단위폐포일양화탄미산량(DLCO/VA)균현저고우대조조,차이균유통계학의의(P <0.05)。결론:보충유생소D 가제고 COPD 환자골밀도,강저염증인자수평,병개선폐공능。
Objective: To explore the effect of vitamin D supplement on bone mineral density and inflammatory factors in patients with COPD. Methods: 350 patients COPD were randomly divided into the observation group(n=175) and the control group(n=175). They al received the conventional treatment, but the patients in the observed group were given the treatment of Calcitriol Soft Capsules (0.25 μg·d-1). The difference of the bone mineral density, inflammatory cytokines and pulmonary function were compared between the two group after treatment for 3 months. Results: ① After treatment, the levels of 25 (OH) D, serum calcium, bone mineral density and T values in the observation group were significantly higher than those of the control group, while the levels of serum phosphorus, TNF- α and IL-6 in the observation group were significantly lower than those of the control group(P<0.05). ② After the treatment, the FEV1, FEV1/FVC and DLCO/VA in the observation group were significantly higher than those of the control group(P<0.05). Conclusion: Vitamin D supplement could improve bone mineral density, decrease the level of inflammatory factors, and improve lung function in patients with COPD.
